功能性解剖单侧唇裂整复同期鼻畸形矫正术的应用研究

摘　　要：目的研究单侧唇裂修复时进行功能性解剖口轮匝肌重建并同期进行鼻畸形矫正术的效果,探讨其对患者术后唇部功能、外形的恢复以及减少术后鼻畸形再次发生的影响。方法选取从2008年3月到2010年3月在我科手术的单侧唇裂合并有鼻畸形的患者109例,患者手术年龄为3～10个月,平均5个月。术式采用交大法加口轮匝肌功能性重建法,同时鼻畸形修复时采用Tajima切口,术后一周佩戴用鼻模6个月,分别于术后1个月、3个月、6个月、12个月、24个月复诊。结果根据鼻孔的对称性、鼻翼是否扁平、鼻小柱是否倾斜、鼻尖是否挺立等项拍照后判断疗效,分为良中差3级,其中良70例,中35例,差4例。结论在单侧唇裂修复时进行口轮匝肌功能性重建同期鼻畸形矫正术,可取得患者唇鼻部术后较满意的效果,减少再次手术的可能及后期手术的难度。Objective To study the unilateral cleft lip repair, the functional anatomy of the orbicularis oris muscle reconstruction and the effect of the nasal deformity correction surgery during the same period, and to explore the impact of its happening again on right patients lip function, shape recovery and nasal deformity reduce. Methods Selecting 109 patients with unilateral cleft lip joint nasal deformity in the department from March, 2008 to March, 2010. Patients' age at surgery was 3 to 10 months, with an average of five months. Surgical cross-Dafa plus the orbicularis oris muscle function reconstruction method was used while the nasal deformity repairs using Tajima incision. Patients should wear nasal mold for 6 months from one week after operation.1 month, 3 months, 6 months 12 months and 24 months referral should be taken. Results Through photographing whether the nostrils is in symmetries, nose is flat, columella is tilt, nose tip stand or not, efficacy was determined, and differential is divided into 70 good cases, 35 middle cases and 4 poor cases. Conclusion Through orbicularis oculi muscle functional reconstruction in the correction with nasal deformities surgery in unilateral cleft lip repair, postoperative satisfactory results can be obtained, and reduce the possibility of secondary operation and the difficulty of post operation.
