中老年人下睑眼袋的个性化修复术临床分析

摘　　要：目的:探讨个性化修复术在中老年人下睑眼袋处理中的应用效果。方法:我院2012年5月至2013年5月收治100例中老年人下睑眼袋患者,行皮瓣法40例、皮肌瓣法28例、皮瓣+肌瓣法20例、结膜入路法12例个性化修复术,回顾临床资料。结果:本组患者均顺利完成手术,行平均6个月随访示,皮瓣法下睑凹陷2.5%(1/40),早期轻度下睑外翻5%(2/40);皮肌瓣法早期轻度下睑外翻8.3%(1/28);皮瓣+肌瓣法早期下睑外翻10%(2/20);结膜入路法下睑细小皱纹增多17.9%(5/28)。结论:中老年人下睑眼袋依据特点行个性化修复,如皮瓣法在无禁忌症的一切下睑眼袋矫正中均可使用,皮肌瓣法适合眶脂肪疝出型等脂肪组织过多者,皮瓣+肌瓣法有较广适应症,结膜入路法适合无皮肤松弛或单纯眶脂肪移位、膨出者,可获得理想预后,不良事件率低。Objective To study the application effect of individualized prosthetics in middle-aged and elderly patients of lower eyelid blepharoplasty. Methods Selecting 100 cases of middle-aged and elderly patient s with lower eyelid blepharoplasty admitted to the hospital from May 2012 to May 2013,row method of skin flaps were used in 40 cases,skin flaps in 28 cases,skin + myocutaneous flap repair of law through law in 20 cases,and conjunctiva and other personalized in 12 cases. Clinical data was reviewed. Results The operatio ns were successfully completed. 6-month follow-up visit was done. There were 2.5%(1/40)lower eyelid sunk through skin flap,5%(2/40)early light lower eyelid ectropin,8.3% skin flap slightly lower eyelid ectrop in,10%(2/20)early lower eyelid ectropin through skin flap+muscle flap,and 17.9%(5/28)increased small lower eyelid wrinkles through conjunctival approach method. Conclusion Lower eyelid pouch of middle-aged and elderly cases are under personalized repair,such as skin flap law can be used in no taboo card of all lower ey elid pouch,skin muscle flap law is suitable for orbital fat hernia out type,and too much fat organization,sk in flap + muscle flap law has more wide adapted card,conjunctival into road law is suitable for no skin relaxa tion or simple orbital fat shift and expanded cases. This can obtain ideal prognosis and reduce bad event rate.
