淋巴细胞免疫治疗不明原因复发性流产妊娠结局分析

摘　　要：目的探讨不明原因复发性流产患者采用淋巴细胞免疫治疗后的临床疗效,分析流产次数、患者年龄与妊娠结局的关系。方法不明原因复发性流产患者604例,于孕前、孕后采用淋巴细胞皮内注射法进行免疫治疗,观察并分析其妊娠结局。结果妊娠成功者420例,成功率84.67%,再次流产者76例,流产率15.33%,其他108例(现孕周<28周4例,未孕40例,失访56例,异位妊娠4例,不明原因胎死宫内1例,胎儿畸形引产3例);流产次数≤3次者治疗后妊娠成功率明显高于流产次数>3次者(P<0.05);年龄≤35岁者妊娠成功率明显高于年龄>35岁者(P<0.05)。结论淋巴细胞主动免疫治疗不明原因复发性流产的疗效肯定,治疗后妊娠成功率高,建议连续自然流产2次或以上者及早干预治疗。Objective To investigate the effect of lymphocyte immunotherapy on the patients with unexplained recurrent spontaneous abortion(URSA) and to analyze the relationship of abortion time and patients’ age with the outcomes of URSA after lymphocyte immunotherapy.Methods There were 604 URSA accepted lymphocyte injection before and after their pregnancy.Pregnancy outcomes were assessed.Results 420 cases were successful from 508 cases of pregnancy, and the success rate of pregnancy was 84.67%,76 cases underwent spontaneous abortion once again,and the abortion rate is 15.33%,108 cases are others(4 cases were in pregnancy within 28weeks,40 cases were unpregnant,56 cases were lost,4 cases were ectopic pregnancy,1 cases were fetal death,3 cases were fetal malformations).The pregnancy success rate of abortion time less than or equal to 3 was significantly higher than that of abortion time more than 3(P &lt;0.05). The pregnancy success rate of age group under 35 was significantly higher than the age group above 35(P &lt;0.05).Conclusion Lymphocyte immunotherapy has curative effect on URSA, and the pregnancy success rate is obviously increased after treatment.URSA patients should be interfered and treated as early as possible.
