超声内镜在食管胃结合部腺癌术前分期中的临床应用

摘　　要：目的分析超声内镜(EUS)检查对食管胃结合部腺癌术前TN分期的准确性,评价其临床应用价值。方法接受外科手术切除的91例经病理证实且.临床资料完整的食管胃结合部腺癌患者,均于术前进行了EUS检查,结合术后病理分期进行对比分析。结果 EUS对食管胃结合部腺癌术前T分期的准确率分别为T_1 95.6%、T_2 91.5%、T_3 85.7%、T_4 90.1%,总的准确率为81.3%;EUS对食管胃结合部腺癌淋巴结转移判断的敏感度较高,分别为No 78.3%和N^+89.7%,总的准确度为86.8%,对于较大淋巴结(直径>1 cm)的判断较为准确,对于远处转移的淋巴结有局限性。结论 EUS可准确判断食管胃结合部腺癌的TN分期为手术方案的制订提供依据。Objective To explore the clinical value of endoscopic ultrasonography（EUS） in the preoperative tumor and nodal（TN） metastasis staging of adenocarcinoma of esophagogastric junction（AEG）.Methods Ninety-one pathologically proven AEG patients with full clinical data,who had received preoperative EUS check and surgical resection,were studied,together with the contrastive analysis of their postoperative pathologic staging.Results The accuracy of EUS in determining the T stage of AEG were T₁ 95.6%,T₂ 91.5%,T₃ 85.7%,T₄ 90.1%,,respectively.The total accuracy of T staging with EUS was 81.3%.The sensitivity of EUS for N staging was higher and it was N₀ 78.3%,N⁺ 89.7%,respectively.The total accuracy of N staging with EUS was 86.8%.It was easy for EUS to determine the lymph node metastasis status of relatively large lymph node whose diameter was larger than 1.0 cm,but as to the distant lymph node EUS was helpless.Conclusion EUS is higher in accuracy degree and can be applied to provide guidancefor working out rational operation methods.
