摘 要:目的探讨CD34、CD117和DOG1在胃肠道间质瘤(gastrointestinal stromal tumors,GIST)中的表达及其诊断价值。方法采用免疫组织化学SP法对2006年6月到2012年11月收治并手术治疗的GIST患者(66例)及非GIST患者(35例)的肿瘤标本进行CD34、CD117和DOG1蛋白表达检测。比较两组患者上述蛋白表达是否存在差别以及与GIST临床病理特征间的关系。结果 GIST组患者CD34、CD117及DOG1阳性率分别为71.2%,93.9%和90.9%,非GIST组分别为9.1%,8.6%和11.4%,组间比较差别均有统计学意义(P<0.05);GIST患者年龄、性别、肿瘤大小、肿瘤部位、组织学类型及危险度分级等临床特征与CD34、CD117及DOG1表达无相关性(P>0.05)。CD117、DOG1及两组联合检测判断GIST的敏感性分别为0.94(95%CI:0.85~0.98)、0.91(95%CI:0.81~0.97)和0.86(95%CI:0.76~0.94);特异性分别为0.91(95%CI:0.77~0.98)、0.89(95%CI:0.73~0.97)和1.00(95%CI:0.90~1.00),诊断的ROC曲线下面积分别为0.88,0.84和0.97。结论 GIST患者CD34、CD117及DOG1表达与临床病理特征无显著相关性;联合检测CD117和DOG1可提高对GIST诊断的准确度。Objective To evaluate the diagnosis value of CD34,CD117 and DOG1 expression in the gastrointestinal stromal tumors( GIST). Methods Sixty- six GISTs patients and 35 non- GIST cases from Jun2006 to Nov 2012 were retrospectively analyzed. The expression status of CD34,CD117and DOG1 were evaluated by immunohistochemistry in the included patients. The relationship between CD34,CD117and DOG1 expression and clinical characteristics were analyzed. Results The positive expression rates were 71. 2%,93. 9% and 90. 9% in CD34,CD117and DOG1of GIST patients,and 9. 1%,8. 6% and 11. 4% of non-GIST subjects,respectively. The positive expression rate of the CD34,CD117and DOG1were significantly higher in GIST group than that in non- GIST group( P < 0. 05). There were no association between the CD34,CD117,DOG1expression level and clinical characteristics in patients with gastrointestinal stromal tumor( P >0. 05). The combined diagnostic sensitivity and specificity by detection of CD117and DOG1 were 0. 86( 95%CI: 0. 76 ~0. 94) and 1. 00( 95%CI: 0. 90 ~1. 00),respectively. The diagnostic area under the ROC curve was 0. 97. Conclusion No association between the CD34,CD117,DOG1expression and clinical characteristics was found in patients with gastrointestinal stromal tumor. Combined detection of CD117and DOG1had significant diagnosis value in patients with GIST.
