超声内镜引导下圈套切除术治疗食管平滑肌瘤

摘　　要：目的探讨超声内镜引导内镜下圈套切除术治疗食管黏膜肌层平滑肌瘤的安全性及临床效果。方法应用超声内镜小探头检查明确的23例食管黏膜肌层平滑肌瘤行圈套切除术治疗,记录并发症、术后病理类型及随访情况。结果 23例患者经上述治疗均完整切除,术中基底部渗血2例,经氩等离子凝固术后出血停止,无穿孔发生,术后复查胃镜所有病例创面愈合良好,无食管狭窄等并发症发生,病理均提示为起源于黏膜肌层的平滑肌瘤,与超声内镜诊断结果一致。19例患者术后1年行胃镜检查,无复发及残留,无食管狭窄,所有患者无胸骨后疼痛、吞咽困难等不适。结论经超声内镜检查初步确诊的食管黏膜肌层平滑肌瘤行内镜下圈套切除术是安全有效的微创治疗手段。Objective To investigate the safety and value of endoscopic trepanned resection( ETR) for treating esophageal leiomyomas guided by endoscopic ulrasonography( EUS). Methods ETR assisted with EUS was performed on 23 cases with leiomyoma of submucous membrane of esophagus. The rates of complications and follow- up investigation were recorded. All resected specimens were remained for histopathological examination. Results 23 lesions were removed completely by ETR. Except for 2 case with bleeding during the procedure and then cured by endoscopic therapy,there was no other complication occurred. No esophageal stenosis was found after operation. All lesions were leiomyoma origin from lamina muscularis muscosae,which were consistent with EUS. 19 patients were followed up for 1 year and there was no relapse,residue and Stenosis.Conclusion EUS guided ETR is a safe and effective minimally invasive therapy for the treatment of esophageal leiomyomas.
