后腹腔镜手术在治疗肾上腺嗜铬细胞瘤中的应用

摘　　要：目的:探讨后腹腔镜手术在治疗肾上腺嗜铬细胞瘤中的应用效果.方法:回顾性分析2005年7月~2011年7月间应用后腹腔镜手术治疗35例肾上腺嗜铬细胞瘤患者的临床资料.结果:35例手术全部获得成功,无一例改开放手术,手术时间为50~240 min,平均95 min,出血量为20~500 ml,平均85 ml,1例术后予输血2个单位,术后平均住院6 d,切除肿瘤大小为2~6 cm,平均3.5 cm,术后并发感染1例,术后病理报告均为嗜铬细胞瘤,平均随访12个月,有1例复发,行二次手术.结论:后腹腔镜下手术治疗肾上腺嗜铬细胞瘤视野清晰,暴露清楚,切除肿瘤完整,能有效的保留正常肾上腺组织,比传统开放手术有着恢复快、创伤小、出血少、术中血压波动较小、手术受腹腔干扰少及患者更容易接受等特点,肿瘤大小的选择取决于术者的操作技术水平,一般小于5 cm.Objective:To study the application effect of laparoscopic surgery in the treatment of adrenal pheochro- mocytoma.Methods:A retrospective analysis was made of 35 patients with adrenal pheochromocytoma receiving ret- roperitoneal laparoscopic surgery in our hospital between 2005.7and 2011.7.Results:All the 35 operations were successful,and none was converted to open surgery.The mean operation time was 95 min,ranging from 50min to 240min.The mean blood loss was 85ml,ranging from 20ml to 500ml.one case received blood transfusion postop- eratively.The mean hospital stay was 6days.The mean diameter of tumor was 3.5cm,ranging from 2cm to 6cm. One case was complicated with infection postoperatively.All the specimens were confirmed pheochromocytoma by pathology.All the cases were followed up for 12 months and one case had recurrence and undergone secondary surgi- cal treatment.Conclusions:Retroperitoneal laparoscopic surgery for adrenal pheochromocytoma has clear field and ex- posure,and could remove the tumor completely and preserve the normal adrenal tissue.Compared with open surger- y,Retroperitoneal laparoscopic surgery has the advantage of rapid recovery,minimal injury,less bleeding,little fluctation of blood pressure during operation,fewer interfences from abdominal cavity and better acception.The se- lection of tumor depends on the operative skills.
