摘  要:目的评估红细胞指数鉴别婴幼儿中间型α地中海贫血(Hb H病)和缺铁性贫血患儿的作用。方法 77例Hb H病及108例缺铁性贫血婴幼儿纳入研究。采用7种公式计算红细胞指数,分析其对Hb H病的预测敏感度、特异性和Youden's指数。结果 Hb H病红细胞比缺铁性贫血高,红细胞平均体积、红细胞平均血红蛋白比后者低,两组患儿的血红蛋白水平差异无显著意义。7种指数中,Green and King指数(GKI)对Hb H病的预测敏感度(90.7%)及Youden's指数(80.3%)最高。结论红细胞指数计算公式可应用于鉴别Hb H病和缺铁性贫血,其中GKI指数可靠性相对较高。Objectives To evaluate the reliability of RBC indices in discriminating between Hb H disease and iron deficiency anemia (IDA) in infants.Methods Seventy-seven patients with Hb H disease and 108 patients with IDA were enrolled in this study. RBC indices were calculated by 7 kinds of formulas. Sensitivity, specificity and Youden's index of these RBC indices for predicting Hb H were also evaluated. Results Comparing the patients' RBC parameters between Hb H disease and IDA groups, high RBC levels were found in the former while highe MCV and MCH in the latter. However, there was no significant difference in Hb level between the two groups. Among seven RBC indices, Green and King Index (GKI) showed the highest sensitivity (90.7%) and Youden's index (80.3%). Conclusion Seven kinds of formulas for RBC index calculation can be used to discriminate between Hb H disease and IDA, and GKI index appears most reliable among them.
