摘 要:目的探讨黏蛋白Muc-5ac基因和多重耐药因子3(MDR3)在胆囊结石形成中的作用及其相互关系。方法应用免疫组织化学法和逆转录聚合酶链反应(RT-PCR)技术检测Muc-5ac和MDR3在36例胆囊结石、20例胆囊炎和12例正常胆囊的胆囊黏膜和肝组织中的表达。结果 Muc-5ac黏蛋白在胆囊结石和胆囊炎的胆囊黏膜中表达的阳性率分别为66.7%(24/36)和40%(8/20),显著高于正常对照组33.3%(4/12)(P<0.01),Muc-5ac黏蛋白表达的阳性率在胆囊结石组又显著高于胆囊炎组(66.7%vs 40%)(P<0.05)。胆囊结石的肝组织中MDR3 mRNA的表达量(1.14±0.31)极显著低于胆囊炎的肝组织(1.89±0.83)和正常肝组织(1.97±0.71)(P<0.01),但MDR3 mRNA的表达量在胆囊炎的肝组织中与正常肝组织之间无显著性差异(1.89±0.83 vs 1.97±0.71)(P>0.05)。胆囊结石的胆囊黏膜中,Muc-5ac蛋白阳性表达者,其肝组织中MDR3 mRNA表达量明显减少,与Muc-5ac蛋白阴性表达组相比,MDR3 mRNA的表达量在两组间差异极显著(0.82±0.17 vs 1.45±0.24)(P<0.01)。结论 Muc-5ac基因参与了胆囊结石形成的过程。在胆囊结石形成的后期阶段,MDR3与Muc-5ac基因相互作用,共同影响胆囊结石的形成。Objective To evaluate the function and relationship between mucin gene (Muc-5ac) and multidrug resistance 3 (MDR3) in cholesterol calculus formation. Methods The expressions of Muc-5ac and MDR3 in mucosa and liver tissue of 36 patients with cholelithiasis, 20 patients with cholecystitis and 12 patients with normal gallbladder were measured respectively with immunohistochemical stains and reverse transcriptionpolymerase chain reaction (RT-PCR). Results The positive rates of Muc-5ac in mucosa were siginificantly higher than those of in mucosa of patients with cholelithiasis(66.7%) and cholecystitis(40%) than those of in mucosa of patients with normal gallbladder(33.3%)(P<0.01), and also higher than those of in mucosa of patients with cholelithiasis 66.7%) than those of in mucosa of patients with cholecystitis(40%)(P<0.05). The mRNA expressions of MDR3 in liver were siginificantly lower in patients with cholelithiasis(1.14±0.31) than those of in liver of patients with cholecystitis (1.89±0.83) and with normal gallbladder(1.97±0.71) respectively. However, there were no difference between the cholecystitis group and the control group (1.89±0.83 vs 1.97±0.71 ) (P>0.05). The MDR3 mRNA of liver in Muc-5ac positive in mucosa with cholelithiasis were siginificantly lower than those of in Muc-5ac negative group(0.82±0.17 vs 1.45±0.24)(P<0.01). Conclusion Muc-5ac may be associated with the whole process of cholesterol calculus formation. MDR3 and Muc-1 interacted to influence the formation of cholelithiasis in the later period.
