血清PSA正常的进展型前列腺癌的诊治(附3例报告)

摘　　要：目的探讨血清PSA正常的进展性前列腺癌的诊断和治疗,提高对PSA正常的进展型前列腺癌的准确认识并提供临床正确诊治此类疾病的参考。方法对3例病人的病历资料,进行回顾分析,并对PSA正常的进展型前列腺癌的原因发生机制进行探讨。结果 3例病人的PSA都小于2 ng/ml,病理证实均为前列腺癌。1例病人病情进展快,但2例联合全雄激素阻断和多西紫杉醇化学治疗能延长病人生存期。结论血清PSA正常的前列腺癌诊断依赖于经直肠B超穿刺活检,治疗时应当注意联合多西紫杉醇化学治疗。Objective To investigate the diagnosis and treatment for advanced prostate cancer whose serum level of prostate-specific antigen(PSA) was normal.To improve the accurate understanding of the advanced prostate cancer with normal serum level of PSA,and provide the correct methods of clinical diagnosis and therapy as reference. MethodsThe clinical data of 3 cases of advanced prostate cancer with normal PSA were retrospectively analyzed.The pathogenesis of the advanced prostate cancer with normal serum level of PSA were discussed.ResultsThree patient's PSA were less than 2 ng /ml,but the results of the pathology confirmed the diagnosis of the advanced prostate cancer.One patient's disease progressed rapidly,but the combination with total androgen deprivation therapy and chemotherapy of docetaxel for two patients could prolong their life.ConclusionsTransrectal ultrasound prostate biopsy helps the diagnosis of advanced prostate cancer with normal PSA,and we should pay more attention to the chemotherapy with docetaxel in the treatment.
