摘  要:目的总结利用血管外科技术提高腹膜后肿瘤(RPT)完整切除率的经验。方法回顾性分析2005年1月~2011年1月11例累及腹膜后大血管的RPT的患者资料,术中采用联合受累血管的切除重建使肿瘤均获得完整切除。结果无手术死亡。9例患者获得随访,随访时间3个月至5年,重建血管通畅。1例低分化腺癌患者术后3个月死于肿瘤全身转移,1例腹膜后脂肪肉瘤术后复发经2次手术恢复良好,其他患者肿瘤无明显复发。结论应用血管外科技术(包括联合重要血管切除重建、血管腔内隔绝、栓塞等)可显著提高累及大血管的RPT的完整切除率。Objective To summary the exercises of improving the radical resection rate of retroperitoneal tumors(RPT) involving great vessels by vascular surgical techniques. Methods The clinical data of 11 cases with RPT underwent radical resection from January 2005 to January 2011 were analyzed retrospectively. The involved vessels were resected and reconstructed during the operation in order to resect the tumors completely. Results There was no operative death. Nine cases were followed up for a time of 3 months to 5 years, the patency rate of reconstructed vessels was 100%. One case of low differentiated adenocarcinoma died of systemic metastasis after 3 months of operation, while 1 case of retroperitoneal liposarcoma recurred after operation and recovered by secondary operation. No tumor recurrences were found in other cases. Conclusion Vascular surgical techniques, including vascular resection and reconstruction, endovascular exclusion and vascular occlusion, can increase resection rate of RPT involving great vessels.
