霍奇金淋巴瘤综合治疗的临床分析

摘　　要：目的：探讨霍奇金淋巴瘤（ HL）的临床特征、治疗和预后。方法78例HL均采用多柔吡星、博莱霉素、长春碱、氮烯咪胺（ ABVD）方案进行一线化疗。结果全组78例HL男性50例，女性28例，年龄5～72岁，中位年龄29岁，69例（88．5％）单侧或双侧颈部淋巴结受侵犯。化疗联合放疗后CR 66例（84．6％）．49例Ⅰ、Ⅱ期患者中，44例（89．8％）CR，29例Ⅲ、Ⅳ期患者中22例（75．9％）CR。中位随访时间91．5个月（3．2～161．1个月），全组患者中7例复发，11例死于肿瘤进展或复发。全组患者5年无失败生存率和总生存率分别为76．3％和87．9％。Ⅰ、Ⅱ期和Ⅲ、Ⅳ期患者5年无失败生存率分别为83．0％和65．1％，差异有统计学意义，P＝0．029；5年总生存率分别为95．6％和75．0％，差异有统计学意义，P＝0．004。结论 HL以男性多见，颈部淋巴结侵犯最常见，ABVD方案联合放疗治疗HL治愈率高，Ⅰ、Ⅱ期患者生存率明显优于Ⅲ、Ⅳ期患者。Objective To evaluate the clinical characteristics ,treatment and prognosis of Hodgkin lymphoma .Methods 78 Hodgkin lymphoma was initially treated with doxorubicin、bleomycin、vinblastine、and dacarbazine (ABVD).Results Of the 78 patients with Hodgkin lymphoma ,50 were male and 28 were female with a median age of 29 years ( Range:5~72 years ) .69 (88.5%) had unilateral or bilateral cervical lymph nodes involvement .66 achieved complete remission ,44 of 49 patients with stageⅠand stageⅡand 22 of 29 patients with stage Ⅲand stageⅣachieved complete remission .7 relapsed and 11 died of disease progression or relapse .The 5-year failure free survival and 5-year overall survival for all patients were 76.3%and 87.9%respec-tively with a median follow-up of 91.5 months (Range:3.2~161.1 m).The 5-year failure free survival for stageⅠ、Ⅱand stageⅢ、Ⅳwas 83.0%and 65.1%respectively( P=0.029).The 5-year overall survival for stageⅠ、Ⅱand stageⅢ、Ⅳwas 95.6%and 75.0%respectively( P=0.004).Conclusion HL was more common in male,cervical lymph node was the most common involvement site .HL treated with ABVD regimen combined with radiotherapy has achieved high cure rate ,the survival rate of stageⅠ、Ⅱpatients are significantly higher than that of stage Ⅲ、Ⅳ.
