TPF方案诱导化疗对局部晚期鼻咽癌临床疗效的影响

摘　　要：目的：探讨TPF方案诱导化疗对局部晚期鼻咽癌临床疗效的影响。方法对49例晚期鼻咽癌患者进行随机分组，其中23例患者进行TPF方案联合同期放化疗治疗（ TPF组），另26例患者行PF方案诱导化疗加同期放化疗治疗（ PF组），2组均进行2个疗程治疗。治疗完成1周后对2组患者的不良反应进行观察，治疗完成12周后评价并比较其临床疗效。结果①TPF组出现52例次不良反应，Ⅰ～Ⅱ级34例（65．38％）、Ⅲ～Ⅳ级18例（34．62％）；PF组出现34例次不良反应，Ⅰ～Ⅱ级28例（82．35％）、Ⅲ～Ⅳ级6例（17．65％）。 TPF组患者出现不良反应的次数、Ⅲ～Ⅳ级不良反应的发生率，明显高于 PF 组患者，差异具有统计学意义（ P ＜0．05）。②TPF 组 CR （82．61％）高于 PF 组（69．23％），且差异明显（P＜0．05），PF组SD（15．38％）明显高于TPF组（4．35％）（P＜0．05）。其他各项组间差异并不明显（P＞0．05）。结论 TPF方案诱导化疗对局部晚期鼻咽癌临床疗效较好，不良反应可接受。Objective To analyze the efficacy of induction chemotherapy with TPF scheme for locally advanced nasopha -ryngeal carcinoma .Methods 49 patients with locally advanced nasopharyngeal carcinoma were randomly divided into 2 groups:23 cases received induction chemotherapy with TPF scheme followed by concurrent chemoradiotherapy (TPF group),and 26 cases received induction chemotherapy with PF scheme followed by concurrent chemoradiotherapy (PF group).The 2 groups received 2 cycles of induction chemotherapy .Toxicities of the 2 groups were observed 1 week after chemotherapy ,and the efficacy of the 2 groups were evaluated 12 week after chemotherapy .Results ①52 times of side-effects were observed in the TPF group ,including 34 times(65.38%) of grade Ⅰ~Ⅱand 18 times(34.62%)of gradeⅢ~Ⅳ;in the PF group,34 times of side-effects were ob-served,including 28 times(82.35%) of gradeⅠ~Ⅱand 6 times(17.65%)of gradeⅢ~Ⅳ.The total incidence of adverse re-actions and the incidence rate of grade Ⅲ~Ⅳadverse reactions in the TPF group were obviously higher than those of the PF group,there had significant difference between the 2 groups (P<0.05).②In the TPF group,CR and SD were 82.61% and 4.35%,respectively;in the PF group,CR and SD were 69.23%and 15.38%,respectively.There had significant difference be-tween the 2 groups(P<0.05),there had no significant difference in other items (P>0.05).Conclusion Induction chemothera-py with TPF scheme is effective for locally advanced nasopharyngeal carcinoma with tolerable side effects .
