PTEN、P27对HER2阳性乳腺癌新辅助化疗疗效的预测价值

摘　　要：目的：探讨PTEN、P27在HER2阳性乳腺癌含蒽环类新辅助化疗中的疗效预测价值。方法回顾性分析接受含蒽环类新辅助化疗（曲妥珠单抗新辅助靶向治疗患者除外）2个疗程以上、化疗后可手术的113例女性HER2阳性乳腺癌患者，采用免疫组织化学的方法，检测肿瘤原发灶空芯针穿刺活检标本PTEN、P27的表达情况，分析分子表达与新辅助化疗疗效的关系。结果113例接受含蒽环类新辅助化疗的患者中，临床部分缓解66例（58．4％），病理完全缓解14例（12．4％），疾病稳定30例（26．5％），疾病进展3例（2．7％）。 PTEN、P27的阳性率分别为62．8％、70．8％， PTEN与P27共表达率44．2％。多因素分析提示PTEN与P27共表达为HER2阳性乳腺癌蒽环类新辅助化疗疗效的独立预测指标（OR＝3．411，95％CI＝1．212～9．597，P＝0．020）。结论 PTEN与P27共表达可以作为HER2阳性乳腺癌蒽环类新辅助化疗的疗效预测指标，但其预后意义还有待长期随访及前瞻性临床研究来证实。Objective To evaluate the predictive significance of PTEN and P 27 protein in the HER2-positive breast cancer patients treated with neoadjuvant chemotherapy .Methods The clinical data of 113 HER2-positive breast cancer patients were analyzed .All these patients were diagnosed by core needle biopsy as invasive ductal breast cancer and treated with no less than 2 cycles of anthracycline-based neoadjuvant chemotherapy before operation ( patients with Trustuzumab-targeted therapy were excluded ) .Biological markers of PTEN and P 27 were evaluated by IHC in the core needle biopsy specimens .The relationship be-tween the predictive significance and the biomarkers expression status was analyzed .Results Among 113 patients,66(58.4%) cases were partial remission after neoadjuvant chemotherapy ,14(12.3%) pathological complete remission,30(26.5%) stable disease and 3 cases(2.8%) were progression disease after neoadjuvant chemotherapy .The positive expression rates of PTEN and P27 protein were 62.8%and 70.8%respectively.PTEN-P27 co-expression rate was 44.2%.PTEN-P27 co-expression was an in-dependent predictive factor for anthracycline-based neoadjuvant chemotherapy on Multivariate analysis ( OR=3.411,95%CI=1.212~9.597,P=0.020).Conclusion PTEN-P27 co-expression is an independent predictive factor for anthracycline-based neoadjuvant chemotherapy in HER 2-positive breast cancer patients ,however the prognostic significance of PTEN and P 27 protein needs identifications with a long-term follow-up and series of prospective clinical studies .
