保乳手术疗效及其患者生存质量评估

摘　　要：目的探讨保乳手术对老年乳腺癌患者的疗效及其生存质量评估。方法 180例早期乳腺癌患者,其中56例患者行保乳手术,63例患者行标准根治术,61例患者行改良根治术。结果保乳手术组具有住院时间较短、手术时间较短、术中出血量少等优势,与根治术组(标准根治术组+改良根治术组)相比有统计学差异(P<0.0001);三组患者生存率、远处转移、术后复发等方面比较均无统计学差异(P>0.05);保乳术组患者心理因子、躯体因子、精神因子和社会支持因子四个方面均显著优于标准根治术组和改良根治术组患者(P<0.05);术后乳房外观及美容效果保乳术组与标准根治术及改良根治术相比有显著统计学差异(P<0.05)。结论早期乳腺癌患者保乳手术近期疗效较满意,严格把握相关手术指征、规范切除和术后放化疗等是保乳手术成功的关键。Objective To analyze the quality of life and efficacy of elderly breast cancer patients treated with breast -conserving surgery .Methods In the 180 early breast cancer patients ,56 cases received breast-conserving surgery ,63 cases re-ceived standard radical mastectomy and 61 cases received modified radical mastectomy .Results Hospitalization time,operative time and blood loss in breast-conserving surgery group were better than those of the radical surgery group ( standard radical sur-gery group +modified radical mastectomy group ) (P<0.0001);No significant differences were identified among the 3 groups in survival rate,distant metastasis and recurrence rate (P>0.05);psychological factors,physical factors,mental factors and social support factors in breast-conserving surgery group were significantly superior to those of the radical surgery group (P<0.05);Compared with radical surgery group ,breast appearance and cosmetic results had significant differences in breast -conserving sur-gery group (P<0.05).Conclusion For early stage breast cancer patients ,breast-conservative surgery has good efficacy .Strict tumorectomy and auxiliary dissection followed by post-operative adjuvant chemotherapy and radiotherapy is very important .
