高能聚焦超声热疗治疗115例原发性肝癌和转移癌的疗效观察

摘　　要：目的　探讨高能聚焦超声 (HIFU)治疗原发性肝癌及转移性肝癌患者的临床疗效。方法　 115例原发性肝癌及转移性肝癌应用HIFU治疗 ,其中 2 5例原发性肝癌同时行介入化疗 ,并通过彩超观察病灶的变化。结果　本组 115例中显效 2 0例 (占 17% ) ,有效 92例 (占 80 % ) ,总有效率 97% ,无效 3例 (占 2 .6 % )。 90 %以上患者治疗后肝区疼痛缓解或消失。彩超显示 :患者病灶体积缩小 ,血供明显减少或消失 ,腹水逐渐减少或消失。临床症状明显改善 ,如 :食欲改善、体重增加等。血清AFP逐渐下降。结论　HIFU治疗原发性肝癌及转移性肝癌是一种无创、无痛、安全、有效的治疗方法 ,介入化疗后进行HI FU治疗 ,效果更加明显。Objective To explore the therapeutic effect of high-intensity focused ultrasound (HIFU) on primary or metastatic carcinoma of liver. Methods A totalof 115 patients with primary or metastatic carcinoma of liver were given HIFUtreatment. Among these patients, there were 25 with primary carcinoma of liver were given intervention chemotherapy at the same time . Changes of the focus carcinoma were observed via color Doppler ultrasound cardiogram (CDUC). Results CDUC showed that the volume of carcinoma focus in most of cases shrank aftertreatment . Blood supply of carcinoma was reduced remarkably and even disappeared after treatment in most of patients. Clinical symptoms such as ascites diminished or disappeared, appetite improved, and body weight increased in the patients. Serum AFP of them decreased gradually after treatment. Among them, 20 showedhigh effective (high effective rate 17%),92 showed effective (effective rate80%) . The total effective rate was 97%.Only 3 patients showed ineffective (2 6%).And 90% of patients felt relief of hepatic pain after the HIFU treatment. Conclusion HIFU therapy is a high-technological, noninvasive, painless, safe and effective way to treat the primary or metastatic carcinoma of liver. Itcan show more effective if patients recieve intervention chemotherapy at the same time.
