摘 要:目的探讨幽门螺杆菌感染、vacA基因亚型和胃上皮NF-κB蛋白表达在慢性胃痛证型的分布及关系。方法收集100例慢性胃痛患者胃窦黏膜,其中脾胃湿热证57例、肝气犯胃证24例、脾胃虚寒证19例,采用免疫组化方法检测胃黏膜上皮细胞NF-κB P65蛋白,特异引物PCR分析vacA基因亚型。结果 100例标本中,Hp感染率为81%,vagA基因亚型sla阳性率90.1%,m2阳性率85.2%;Hp定植密度与NF-κB P65表达呈正相关;与阴性组比较,s1a+和m2+提高了NF-κB P65表达(P均<0.01),Hp定植密度、vacA基因亚型、NF-κB P65蛋白在3种中医证型中差异无统计学意义。结论 vacA基因以s1a/m2亚型为主,Hp感染增加了NF-κB P65蛋白的表达,但Hp定植密度、vacA基因亚型、NF-κB P65蛋白不能作为中医证型的预测指标。Objective To investigate the distribution of Helicobacter pylori(Hp) infections, vacA gene subtypes and gastric epithelial nuclear factor-κB(NF-κB) expression in chronic stomachache(CS) patients with different TCM syndromes, and explore the relationship among each other. Methods The specimens of antral gastric mucosa of 100 CS patients were collected, including 57 cases of spleen-stomach dampness-heat syndrome, 24 cases of liver Qi attacking stomach syndrome and 19 cases of spleen-stomach deficiency-cold syndrome. The gastric epithelial NF-κB P65 protein was determined by immunohistochemistry, and the vacA gene subtypes were detected by polymerase chain reaction sequence specific primer. Results In 100 specimens, the Hp infection rate was 81%, and the positive rate of signal allele sla, middle-region allele m2 in vacA gene was 90.1%, 85.2% respectively. The Hp colonization density was positively related to NF-κB P65 protein expression. The NF-κB P65 protein expression was significantly higher in sla+or m2+ group than that of negative group(P<0.01). There was no significant difference in Hp colonization density, vacA gene subtypes and NF-κB P65 protein expression among the three syndromes. Conclusion The main subtypes in vacA gene are sla / m2, the Hp infection can increase the expression of NF-κB P65 protein, but Hp colonization density, vacA gene subtypes and NF-κB P65 protein expression couldn't be used as prediction indexes of TCM syndromes.
