摘　　要：目的探讨食管癌、癌旁组织中P53、MDR-1基因的表达及相关性。方法选择接受手术治疗的50例食管癌患者的癌组织蜡块,均采用免疫组化ABC法及逆转录酶-聚合酶链反应的方法对食管癌、癌旁组织中P53、MDR-1基因进行检测。结果 P53阳性率在食管鳞癌为68%(34/50),癌旁正常黏膜为54%(27/50),癌旁非典型增生为70%(35/50);食管癌组织中P53的表达与MDR-1的表达有高度相关性;高度分化食管癌组织中MDR-1的表达与中度分化比较差异有统计学意义(P<0.01);中度分化组织中MDR-1的表达与低度分化比较差异有统计学意义(P<0.01);低度分化组织中MDR-1的表达与正常组织比较差异有统计学意义(P<0.01)。结论 MDR-1基因及突变型P53在食管癌、癌旁组织中均呈高水平表达,癌旁组织中MDR有表达,P53基因突变早于组织形态学改变。Objective To explore the expressions of P53 and MDR-1 gene in esophageal carcinoma,paracancerous tissues and the correlation.Methods 50 cases of esophageal cancer patients were selected.ABC immunohistochemistry and reverse transcriptase polymerase chain reaction method were used to examine the expressions of P53 and MDR-1 in esophageal carcinoma and paracancerous tissues.Results Positive expression rate of P53 in the esophageal squamous cell carcinoma was 68%(34 /50),in adjacent normal mucosa was 54%(27/50),in paracancerous atypical hyperplasia was 70%(35/50).There was high degree of correlation between P53 and MDR-1 expression in esophageal cancer tissue.There was statistically significant difference in expression of MDR-1 between highly and middlely differentiated esophageal cancer tissues(P<0.01).There was statistically significant difference in expression of MDR-1 between low differentiated esophageal cancer tissue and normal tissue(P<0.01).Conclusions There are high levels of expressions of MDR-1 gene and mutant P53 in esophageal cancer,there is MDR-1 expression in adjacent tissues,P53 gene mutation is before morphological changes.
