原发性结外型淋巴瘤243例回顾分析

摘　　要：目的探讨原发性结外型淋巴瘤的临床特点。方法回顾分析中大大学附属第一医院血液科2004年5月至2008年6月住院诊治的243例原发性结外型淋巴瘤的临床资料。结果原发性结外型淋巴瘤男性发病多于女性,30岁～69岁为发病高峰年龄,占首位的是咽淋巴环淋巴瘤,其次为胃肠道淋巴瘤和脾淋巴瘤。结外型淋巴瘤多为非霍奇金淋巴瘤,咽淋巴环淋巴瘤病理类型以T细胞或NK/T细胞为多见,胃肠道恶性淋巴瘤以B细胞淋巴瘤为多见,分期以Ⅰ期Ⅱ期为主,发热在结外型淋巴瘤中均较多见,脾淋巴瘤较咽淋巴环淋巴瘤及胃肠道淋巴瘤更易出现发热。结论应重视原发性结外型淋巴瘤的临床特点,以便减少其误诊与漏诊率,进一步提高其诊治水平。Object To analyze the characteristic of clinical manifestation of primary extranodal non-Hodgkin lymphoma (PENL).Method Retrospective analysis clinical data of all 243 cases of PENL who were diagnosed with pathological examination in our hospital from May 2004 to June 2008.Result The incidence of PENL was 36.1%(243 / 674) of the malignant lymphoma(ML) in the same period and the incidence in the males is higher than that of females.The median of peak age incidence is between 30-year-old to 69-year-old age.The Waldeyer's ring was the most common extranodal site and was seen in 59 of 243(24.3%) patients,followed by the gastrointestinal tract lymphoma(17.3%) and splenic lymphoma(15.6%).The most frequent pathological type of Waldeyer's ring lymphoma is T cells or NK/T cells, while that of gastrointestinal tract lymphoma is B cells.Patients with Waldeyer's ring lymphoma or splenic lymphoma are mostly in early(Ann Arbor stagesⅠ-Ⅱ) disease.Fever is common seen in primary extranodal non-Hodgkin lymphoma. Patients with splenic lymphoma are more vulnerable to have fever than who with Waldeyer's ring lymphoma and gastrointestinal tract lymphoma.Conclusion We Should pay attention to the characteristic of clinical manifestation of PENL, in order to reduce the misdiagnosis and missed diagnosis rate,to further improve its diagnosis and treatment.
