卒中单元内早期综合康复训练对急性脑卒中患者的疗效观察

摘　　要：目的研究卒中单元内综合康复训练对急性脑卒中患者的疗效。方法将116例急性脑卒中患者随机分为对照组和研究组各58例,对照组给予常规治疗,研究组在常规药物治疗基础上给予卒中单元内综合康复训练,观察生活能力评定(Bar-thel指数)、神经功能评定(NIHSS)、运动功能评定(FMA)及各种并发症的发生率等指标,分析卒中单元内综合康复训练的效果。结果治疗前两组的Barthel指数、NIHSS评分、FMA评分的差异均无统计学意义(P>0.05);治疗1个月后,研究组各项评分显著优于对照组,差异均有统计学意义(P<0.01);压疮、泌尿系感染、肩手综合征、抑郁症、吞咽障碍、关节挛缩等并发症发生率较对照组低(P<0.05)。结论卒中单元内早期综合康复训练能明显提高患者的日常生活能力,减少神经功能缺损,促进瘫痪肢体功能恢复,减少脑卒中并发症的发生,提高脑卒中的近期治疗效果。Objective To investigate the therapeutic effect of multimodality rehabilitation training in the stroke unit of acute stroke patients.Methods 116 patients with acute stroke were randomly divided into the observation group(n=58) and the control group(n=58).The control group received the routine treatment,while the experimental group,based on routine treatment,was provided with multimodality rehabilitation training in the stroke unit.Two groups were observed and compared in terms of ADL(Barthel Index),NIHSS and FMA before and after treatment,and the complication rates of two groups were also recorded,to analyze the effect of multimodality rehabilitation training in the stroke unit.Results The figures of ADL(Barthel Index),NIHSS and FMA before treatment had no statistical significance between the two groups(P>0.05);after one month of treatment,the figures in the observation group were superior to those of control group and the differences had statistical significance(P<0.01).In observation group,the rates of complication,such as bedsore,urinary infection,hand-shoulder syndrome,depression,dysphgia,contracture of joints,were lower than those of the control group(P<0.05).Conclusion Multimodality rehabilitation training in the stroke unit can improve patients' activity of daily living,relieve neurologic impairment and promote functional recovery of paralyzed limbs.It can also reduce complication rate of stroke and improve therapeutic effect of stroke.
