女性性工作者与普通人群子宫颈HPV感染率及分型比较

摘　　要：目的研究女性性工作者与普通人群人乳头瘤状病毒(HPV)感染率和分型比较。方法在社区卫生站获得每周商业性性行为次数超过5次且从业时间达半年以上的女性性工作者45例(17～51岁,平均年龄27.4岁)与中国医科大学附属第一医院体检中心普通人群44例(25～55岁,平均年龄34.2岁)的子宫颈脱落细胞,用基因芯片导流杂交技术进行HPV DNA的检测与分型比较。结果女性性工作者HPV感染率37.8%(17/45),在感染者中单一感染率82.4%(14/17),双重感染率11.8%(2/17),三重感染率6.0%(1/17)。最常见的亚型为HPV16,其余依次为HPV33、53、18、6、39、52、66。普通人群HPV感染率11.4%(5/44),均为单一高危型HPV亚型感染,类型为HPV16、33、58、68,其中以HPV33最常见,余各亚型检出率均为20%。结论女性性工作者在感染率和检出亚型明显高于普通人群。本次所有受检者HPV流行亚型为HPV16、33,这与流行于中国北方的HPV16与长江以南的HPV58有地域上的差异。Objective To investigate the comparison of infection rate of HPV and different genotypes of the cervical HPV between female sexual workers and general people. Methods We got the cervical cells from 45 female sexual workers (17～51 years,27.4 years in average)who have more than 5 times commercial intercourses each week and provide sexual service more than half year that we collected in the community health department and 44 general people (25～55 years,34.2 years in average)who come to the First Hospital of China Medical University . HPV DNA of cervical samples was detected and different genotypes were compared by the gene chip flow-guiding hybridization technique. Results HPV infection rate of female sexual workers is 37.8% (17/45),among them,single infective rate is 82.4%(14/17), double infective rate is 11.8% (2/17),triple infection rate is 6.0% (1/17),the most popular genotype is HPV16,others in down order are HPV33,53,18,6,39,52,66. HPV infection rate of general people is 11.4%(5/44),all of the infection ones are single high-risk HPV genotype,HPV16,33,53,58,68 were detected and HPV33 is the most common,other genotypes infection rate is account for 20%. Conclusion HPV infection rate and kinds of genotypes of female sexual workers are apparently higher than general women. The prevalent genotypes of all participants in this study are HPV16,33,which are different from HPV16 prevalent in the north of China and HPV58 popular in south of The Changjiang River in region.
