摘　　要：目的总结神经内镜经单鼻孔蝶窦入路切除鞍区肿瘤的经验和改进方法。方法对29例鞍区肿瘤行神经内镜下经单鼻孔蝶窦入路切除。结果肿瘤全切25例,次全切3例(垂体腺瘤1例,颅咽管瘤2例),部分切除1例(脊髓瘤1例),出现脑脊液5例,尿崩症1例,均在1周内恢复,19例激素异常的病例中18例恢复正常,随访3～15月,所有病例症状均有所好转。结论神经内镜经单鼻孔蝶窦入路切除鞍区肿瘤既安全又彻底,且创伤小,患者满意度高。Objective To summary the experience and improvement of excision of the saddle area tumor through single nostrils and sphenoid sinus with neuroendoscopy.Methods 29 cases of patient were performed the operation of excision of the saddle area tumor through single nostrils and sphenoid sinus with neuroendoscopy.Results Complete resection 25 cases,subtotal resection 3 cases(The pituitary adenoma 1 case,Craniopharyngioma 2 cases),partial resection 1 case(chordomas 1 case),cerebrospinal fluid leakage 5 cases,diabetes insipidus 1 case.They were all recovery in a week.18 cases of patients' hormone return to normal in the 19 cases of hormonal abnormalities patients.Followed up for three months to fifteen months,all cases' symptoms improved.Conclusion Excision of the saddle area tumor through single nostrils and sphenoid sinus with neuroendoscopy was a safty,thorough, less injure and high satisfaction.
