摘 要:作者分别应用压宁定(n=15)和甲氰咪胍(n=15),观察它们对肝硬变门静脉高压症患者肝静脉楔压(WHVP)及肝静脉压力梯度(HVPG)的影响。结果发现经周围静脉输入压宁定25mg后WHVP由3.27kPa±0.77kPa降至2.32kPa±0.67kPa,平均下降20.05%(P<0.01);HVPG由2.15kPa±0.23kPa降至1.38kPa±0.35kPa,平均下降35.81%(P<0.01);同时用药前后心率增加41.98%、收缩压下降25.32%,舒张压下降27.01%,差异明显(P<0.01);自由肝静脉压(FHVP)无显著变化(P>0.05)。经周围静脉输入甲氰咪胍400mg后,WHVP从3.36kPa±0.72kPa降至2.85kPa±0.69kPa,平均下降15.18%(P<0.01);HVPG由2.23kPa±0.80kPa降至1.68kPa±0.42kPa,平均下降24.67%(P<0.01);但心率、血压及FHVP等变化均不明显(P>0.05)。甲氰咪胍和压宁定都可以明显降低WHVP和HVPG。压宁定降低门静脉压力的同时,会出现血压下降和心率增快;但甲氰咪胍对心率和血压无明显影响。The authors researched the effects of Ebrantil(n= 15)and Cimetidin(n= 35) on wedged hepatic venous pressure(WHVP) and hepatic venous pressure gradient(HVPG)of the cirrhot-ic patients with portal hypertension respectively. The Ebrantil given intravenously at a dosage of 25mg significantly reduced WHVP from 3. 27kPa ±0.77kPa to 2. 32kPa±0. 67kPa, the average value is 20.02% (P< 0. 01), HVPG is reduced significantly from 2. 15kPa±0. 23kPa to 1. 38kPa±0. 35kPa,the average value is35.81%(P<0.01),at the same time the contract blood pressure is reduced significantly 25. 32% (P<0.01)and diatolic blood pressure is also reduced significantly 27. 01%(P<0.01), the heart rate is increased significantly 41.98% (P<0. 01), but it is not significantly affeeted on FHVP(P >0.05) . The cimetidin given intravenously at a dosage of 400mg significantly reduced WHVP from 3.36kPa±0.72kPa to 2.85kPa±0.69kPa, the average value is 15.18 % ( P < 0.01), HVPG is significantly reduced from 2.23kPa±0. 80kPa to 1.68±0.42kPa, the average value is24.67%(P<0.01), but no signficantly effects on FHVP, blood pressure and heart rate ( P > 0.05).The results reveal that the Ebrantil and Cimetidin could decreased WHVP and HVPG significantly, Ebrantil not only reduced portal pressure but also decreased arterial pressure and increased heart rate, Cimetidin reduced portal pressure but it did not affected significantly on arterial pressure and heart rate.
