摘　　要：目的 研究甲状腺癌组织中CD15抗原的表达情况及其临床意义。方法 应用微波-LSAB免疫组织化学法,检测50例甲状腺癌、45例甲状腺腺瘤、20例癌旁甲状腺和10例正常甲状腺组织中CD15抗原的表达。结果 在甲状腺癌中CD15表达阳性率为68.0％,显著高于甲状腺腺瘤和癌旁甲状腺(42.0％和35.0％,P<0.05),10例正常甲状腺组织中2例呈CD15弱阳性反应。CD15表达与甲状腺癌组织类型无关,在淋巴结转移病例和病理分期Ⅲ～Ⅳ期病例,CD15表达阳性率显著高于相应的无淋巴结转移病例和病理分期Ⅰ～Ⅱ期病例(P<0.05)。CD15阳性的甲状腺癌复发及死亡率显著高于CD15阴性的甲状腺癌复发和死亡率(P<0.05)。结论 CD15表达对甲状腺癌恶性程度判断、生物学行为预测和预后评估是一种有意义的客观指标。Objective To study the expression of CD15 antigen in thyroid cancer and its significance. Methods The expression of CD15 antigen in 50 thyroid cancers, 20 adjacent noncancerous portion, 45 adenoma and 10 normal thyroid tissue were respectively investigated by Microwave-LSAB im-munohistochemical technique. Results The expression rate of CD15 in thyroid cancer was 68.0%, which was significantly higher than 35.0% in adjacent noncancerous portion and 42.0% in adenoma (P<0. 05). CD15 immunoreactivity was weakly presented in 2 of 10 normal tissue. No correlation was observed between expression of CD15 and histological grading of thyroid cancer. The expression of CD15 in lymph node positive and grade Ⅲ-Ⅳ were both higher than that in negative cases and grade Ⅰ-Ⅱ (P<0.05). The percentage of recurrence and death in CD15 positive cases were notably higher than that in negative cases ( P < 0.05). Conclusion The expression of CD15 can be regarded as a parameter for evaluating the degree of malignacy, biological behavior and prognosis of thyroid cancer.
