摘 要:目的 探讨产前超声筛查胎儿先天性心脏病的临床应用价值。方法 应用二维超声对 43 5例高危孕妇进行产前胎儿心脏超声检查 ,探查四腔心、左室流出道、右室流出道、五腔心、主动脉弓等切面 ,并加用彩色多普勒观察心脏血流方向 ,测量血流速度。结果 43 5例高危孕妇于产前共检出 2 1例先天性心脏病 ,检出率为 4.83 % ,远远高于在低危人群中的检出率 ,另外尚有 3例产前漏诊 ,2例产前未做心脏超声 ,产后超声心动图检查确诊 ,1例误诊。结论 应加强产前高危孕妇的胎儿心脏超声检查 ,降低漏诊率 ,尽早确诊 ,尽早采取措施 。Objective To investigate the clinical application value of prenatal ultrasound screening for fetal congenital heart disease (CHD). Methods There were 435 high-risk pregnancies in this study. Four-chamber views, left and right ventricular outflow, five-chamber view and aortic arch view were observed with two-dimension fetal echocardiography. Color duplex ultrasonography was added to evaluate the direction and velocity of the heart blood flow. Results Among 435 high-risk pregnancies, there were 27 fetus with CHD (4.83%), 3 cases with CHD were not detected before delivery, no prenatal ultrasound performed in two cases until post partum and one case was misdiagnosed. Conclusion Fetal echocardiography should be carefully performed on the patients with high risk pregnancies for detecting CHD. The earlier and more accurate to diagnose CHD, the better outcome would be achieved through efficient management of the fetus with CHD.
