急性非淋巴细胞白血病的临床治疗研究

摘　　要：本文对1981～1991年收治的268例急性非淋巴细胞白血病(ANLL),用HOAP组治疗78例,CR率44.87％。HA组治疗68例,CR率48.5％;DA较大剂量化疗组治疗72例,CR率76.38％;DA与HOAP、HA组的疗效差异显著(P<0.005),应用ATRA和DAP组治疗M_3,ATRA组28例,CR率92.85％,DAP组22例,CR率54.54％,两组疗效差异显著(P<0.05)。CR后必需坚持长期多疗程的强化治疗方能延长生存期,提高治愈率。This present study reports the clinical treatment of 268 cases of acute nonlymphocytic Leukemia (ANLL) which were teated in our group from 1981 to 1991. The curative effect of the HOAP, HA and DA regimens on ANLL M1, M2, M4, M5, M6 types was observed respectively. 78 cases were teated with the HOAP regiment and complete remission (CR) was achieved in 35 patients with a CR rate of 44.87%. 68 cases were teated with the HA regimea and 33 had CR with CR rate of 48. 52%, 72 case were teated with tie DA regimen by large doss and 55 had CR with a CR with a CR rate of 76.38%. No obvious diffeace was found between the first two regimens(HOAP. HA) in curative effect(P>0. 05) . However, great difference was seen between the first two regimens (HOAP, HA) and the third regimen (DA) in terms of curative effect(P>0. 005) . Meanwhile, the curative effect of the ATRA and DAP regimens on ANLL M8 type was also studied. 28 cases were teated with the ATRA regimen and 26 had CR rate of 92.85%. 22 cases were teated with the DAP regimen and 12 had CR with a CR rate of 54.54%. Obvious difference was found between the ATRA regimen and the DAP regimen as far as curative effect is coneerned(P< 0.05). It is auggesed that iong-term and intensified teatment with large dose should be given to patients after CR in order to raise the survival rate and the curative rate. Comparison was made with regard to the curative effect on the various subtypes of ANLL. The results show that ANLL M2 and M3 types obtained the test curative effect with their respective CR rate of 97. 14% and 92.82%. During our teatment of ANLL M2, M3, M4 types, 9cases of central nervous leukemia(CNL) were discovered, which indicates that proper methods should be taken to prevent ANLL M2, M3, M4 types under chemotherapy from being attacked by CNL.
