免疫印渍法检测包虫病人抗体及其在临床和流行病学上应用的探讨

摘  要:本文报告了用酶联免疫吸附试验(ELISA)和免疫印渍法(Western Blot,WB)检测包虫病人血清抗体以及在流行病学调查中同时使用两种血清学方法进行抗体检测的结果,WB 法主要是以肉眼观查38kDa抗原带(抗原5)来判定结果。此项试验结果表明,以38kDa 反应区带为诊断依据的免疫印渍法在敏感性和特异性上均低于 ELISA.对一个未知地区进行包虫病的流行病学调查时,使用 ELISA 方法比 WB 法更为合适,因此应当进一步探索提高 WB 诊断价值的途径.The detection of serum antibodies of hydatidosis patients by ELISA and Western blot (WB),which is assessed by the recognition of 38 kDa zone,and the simultaneous use of both serological tests in epidemiological survey showed that of the 47 sera from surgically confirmed hydatidosis patients 40(85.1%)were positive for ELISA and 32(68.1%)were positive for WB,that in field study made on 126“healthy”subjects B-mode ultrasonography and X-ray examination detected 12 cyst-carriers,with 8 ELISA-positives and 8 WB-positives in them,a 100% coincidence rate between the two tests and that in the remaining 114 sub- jects in whom no hydatidosis was detected by B-mode ultrasonography and X-ray examina- tion,28(24.6%)were positive for ELISA and 31(27.2%)were positive for WB,the coinci- dence rate being 71%.The inferiority in sensitivity and specificity of WB,which is based on the recognition of 38 kDa zone,to ELISA demands further exploration of ways to enhance the diagnostic value of WB.
