摘 要:本文对72例COPD肺动脉高压患者氨茶碱静脉负荷剂量、血浆茶碱浓度与平均肺动脉压(mPAP)变化的关系作了初步研究。结果表明,6mg/kg组静注氨茶碱后mPAP下降较用药前有显著差异(P<0.01),用药后30~120分钟血浆茶碱浓度为115.54~79.μmol/L,无严重副作用发生。建议以6mg/kg作为氨茶碱治疗COPD肺动脉高压的静脉负荷量,其维持量于负荷量后2小时左右给予较为适当。The relationship among thedosages of aminophylline, plasma levels oftheophyline and variations of mean pulmo-nary arterial pressure (mPAP) in 72patients with COPD was investigated. Theresults showed that aftera different loadingdosage of aminophylline (6mg/kg, 5mg/kgand 4mg/kg) was administered by intrave-nous injection, mPAP in the 6mg/kg groupwas decreased more significantly (P<0.01)than that in the 4mg/kg group. In the 6mg/kg group, the decreased mPAP periodsustained for 120 min, which was longerthan that in the other 2 groups. The plasmalevels of theophylline in the 6mg/kggroup of patients 30 to 120 min after load-ing dose injected were 115.54-79.04μmol/L, which were higher than that in theothers. Within the 120 min period of obser-vation after the drug was administered nopatients in any of these groups showedsevere untoward effects. According to the results of this experi-ment, we suggest that the 6mg/kg as aloading dose should be advised for thetreatment of pulmonary hypertension inCOPD. The optimum time to give themaintenance dosage should be set within2h after the loading dose. It is necessary to monitor the plasmalevels of theophylline while aminophyllineis administered, so that optimal therapeuticeffects could be achieved without sideeffects.
