摘　　要：目的比较高度近视患者配戴眼镜与晶状体摘出联合Bigbag人工晶状体植入后的视力差别,探讨其对术后患者屈光状态影响的机制。方法选择屈光度为-8.50^-29.00 D的27例(48眼)行透明晶状体摘出联合Bigbag人工晶状体植入术的高度近视患者,比较手术前后最佳矫正视力(best corrected visual acuity,BCVA)变化情况。同时根据模型眼理论计算植入眼内的人工晶状体与相应戴框架眼镜状况下视网膜像放大率(relative magnification,RM)的变化,评价其与视力提高之间的关系。结果人工晶状体植入3个月后BCVA为4.2~5.2(4.811±0.241),与术前相比,两者差异有统计学意义(t=-12.024,P=0.000)。术后BCVA≥4.9~5.2者20眼,术后BCVA为≥4.5~4.9者24眼,BCVA≥4.2~4.5者4眼。术后BCVA≥4.9~5.2者RM变化值(1.228±0.072)与BCVA提高值(0.245±0.165)呈正相关(r=0.461,P=0.041);术后BCVA≥4.9~5.2者术前BCVA和术后BCVA提高程度呈负相关(r=-0.905,P=0.000)。结论高度近视患者行透明晶状体摘出联合Bigbag人工晶状体植入后,视网膜像放大作用是高度近视的术后BCVA改善的重要原因之一,视力提高量与RM变化量有相关性。Objective To compare the differences of visual acuity between lens extraction with Bigbag intraocular lens(IOL)implantation and wearing spectacles in patients with high myopia.Methods Forty-eight eyes of 27 patients with diopter from-8.50 D to-29.00 D underwent clear lens extraction with Bigbag IOL implantation were enrolled,the best corrected visual acuity(BCVA)before and after operation was compared.At the same time the relative magnification(RM)change of retinal image under IOL implantation and wearing spectacles was calculated.The relationship between RM change and BCVA improvement were analyzed.Results BCVA at 3 months after IOL implantation was from 4.2 to 5.2(4.811±0.241),there was statistical difference compared with pre-operation(t=-12.024,P=0.000).The postoperative BCVA equal to or more than 4.9-5.2 was in 20 eyes,more than 4.5-4. 9 was in 24 eyes equal to or more than 4.2-4.5 in 4 eyes.The change value of RM(1.228±0.072)in patients with postoperative BCVA equal to or more than 4.95.2 was positive correlated with increased value of BCVA(0.245±0.165)(r=0. 461,P=0.041).The preoperative BCVA in patients with postoperative BCVA equal to or more than 4.9-5.2 was negative correlated with the increased degree of postoperative BC VA(r=-0.905,P=0.000).Conclusion RM is one of reasons for postoperative BCVA improvement in high myopia after implanting Bigbag IOL.There is a positive postoperative BCVA improvement.
