卒中单元康复小组模式在急性脑卒中的应用

摘　　要：目的研究在卒中单元康复小组模式下急性脑卒中患者早期康复治疗的临床疗效。方法 84例急性脑卒中偏瘫患者随机分为常规模式组(40例)和康复小组模式组(44例)。常规模式组患者给予常规脑卒中的药物治疗,康复模式组在常规治疗基础上于病情稳定48h开始进行卒中单元的肢体运动功能训练。于治疗前及治疗后采用美国国立卫生研究院卒中量表(NIHSS)、简化Fugl-Meyer运动功能量表(FMA)、功能独立性评定表(FIM)、汉密尔顿抑郁量表(HAMD)进行评定。结果治疗前2组FIM评分、NIHSS评分、FMA评分、抑郁评分差异无统计学意义(P均>0.05);治疗后NIHSS评分、抑郁评分均有所下降,FIM评分、FMA评分均有所升高,治疗后康复模式组上述指标的改善程度明显优于常规模式组(P均<0.05)。结论卒中单元康复小组模式能明显提高急性脑卒中患者的功能独立性,有效促进神经功能恢复。Objective To study the clinical curative efficacy of rehabilitation group model of stroke unit in the early rehabilitation therapy of patients with acute stroke.Methods 84patients with acute stroke were randomly divided into the normal model group(40cases)and the rehabilitation therapy group(44cases).The normal model group was given with conventional anti-stroke medicine,and the rehabilitation therapy group was given with conventional anti-stroke medicine plus rehabilitation treatment after 48hours of the stabilization of disease condition.The curative effect of two groups were evaluated by National Institute of health stroke scale(NIHSS),simplified Fugl-meyer assessment(FMA),functional independence measure(FIM) and Hamilton rating scale for depression(HAMD)before and after treatment.Results The NIHSS,FMA,FIM and HAMD between two groups had no statistical meaning before treatment;The NIHSS and HAMD of two groups were lowered and the FMA and FIM of two groups were increased;The above indexes of rehabilitation therapy group were superior to these of normal model group.Conclusion The rehabilitation group model of stroke unit can significantly increase the function independence of patients with acute stroke,and promote the recovery of neurological function.
