超声乳化白内障吸除术治疗Fuchs综合征并发白内障的疗效观察

摘　　要：目的 评价超声乳化白内障吸除联合人工晶状体植入术治疗Fuchs综合征并发白内障的临床疗效.方法 回顾性系列病例研究.选取2008年3月至2011年8月在河北医科大学第三医院眼科确诊为Fuchs综合征并发白内障的患者13例(13只眼),接受超声乳化白内障吸除术联合人工晶状体植入术治疗,观察患者术前、术后视力,眼压,角膜后沉着物(KP),房水闪辉及术中、术后并发症.患者术后随访12 个月.结果 13只眼术后视力均有不同程度的提高,术后1 年最佳矫正视力≥0.6者12只眼.其中最佳矫正视力≥1.0者5只眼.术前术后眼压未见明显波动;术前存在KP患者术后炎症反应无明显增加,术后2～3周 7只眼出现新的KP,3只眼伴房水闪辉,对症治疗后症状消失.术中无严重并发症,术后2只眼后囊膜混浊,1只眼人工晶状体半脱位.结论 超声乳化白内障吸除,联合人工晶状体植入术是治疗Fuchs综合征并发白内障的一种理想的方法,手术不会增加Fuchs综合征的炎症反应.Objective To eveluate the clinical effect of phacoemulsification and intraocular lens implantation in curing Fuchs heterochromic cyclitis accompanied with cataract. Methods It was a retrospective case series study. A total of 13 eyes in 13 patients with Fuchs heterochromic cyclitis complicated cataract from Mar. 2008 to Aug. 2011 in the Third Hospital of Hebei Medical University were included in this study. All of the 13 eyes were undertaken phacoemulsification and intraocular lens implantation. Visual acuity, intraocular pressure, keratic congestion( KP), aqueous flare as well as complications were observed before and after the operation. 12 months follow-up were completed. Results Visual acuity increased in different degrees in all of the 13 eyes. After 12 months,the best corrected visual acuity of 12 eyes are better than or equal to 0. 6,among which 5 eyes are better than or equal to 1. 0; No significant intraocular pressure fluctuation was observed before and after operation; The surgery did not cause more inflammation in patients who had KP before operation while 7 cases with new KP,3 cases with aqueous flare 2-3 weeks after surgery, the symptoms disappeared after treatment. No severe intraoperative complication was observed. 2 cases developed posteriorcapsular opacification and 1 case artificial lens subluxationafter surgery. Conclusion Phacoemulsification and intraocular lens implantation is an ideal surgery and do not increase the inflammation in treating Fuchs heterochromic uveitissimultaneity with cataract.
