术中局部植入缓释氟尿嘧啶对结直肠癌术后早期机体免疫功能的影响

摘　　要：目的研究术中局部植入缓释氟尿嘧啶对进展期结直肠癌术后早期机体免疫功能的影响。方法进展期结直肠癌患者80例随机分为两组,每组40例:A组,行根治性手术,术中局部植入缓释氟尿嘧啶800mg;B组,术中单纯行根治性手术;两组术后治疗相同。另设C组,16例结直肠良性肿瘤局部切除患者作为对照组。结果 A、B组患者的细胞免疫指标(CD3、CD4、CD4/CD8、NKC、IL-2R和T淋巴细胞转化率)及体液免疫指标(sIL-2R和TNF-a),在术后1w、2w、3w明显降低,与术前相比较有统计学差异(P<0.05);但A、B两组同期比较无统计学差异(P>0.05)。C组外周血单个核细胞的分布及体液免疫功能指标在术前、术后相比较无统计学差异(P>0.05),但均比A、B组高,且有统计学差异(P<0.05)。结论术中植入缓释氟尿嘧啶对结直肠癌术后早期机体免疫功能无影响。Objective To study the effects of intro-operative local implant of sustained-release fluorouracil on immunity in early post-operative patients with colorectal carcinoma.Methods Eighty patients with colorectal carcinoma were randomly divided into group A and group B,40 in each group.All patients underwent radical resection.In group A,the patients received intro-operative local implant of 800 mg sustained-release fluorouracil.Two groups received the same postoperative therapy.16 patients in group C,as control group,received simple operations for benign colorectal neoplasm.Results In group A and B,the cellular immune indexes (CD3,CD4,CD3/CD8,NKC,IL-2R,T lymphatic cell transformation rate) and humoral immune index (sIL-2R,TNF-a)statistically decreased on the 1st,2nd and 3rd week after operation,compared with preoperative values respectively(P<0.05),while there were no statistical differences on the same postoperative time between the two groups.In group C,the postoperative peripheral mono-nuclear blood cell distribution and humoral immune indexes had no statistical difference before and after operation(P>0.05),but were significantly higher than those in group A and B respectively(P<0.05).Conclusion Intro-operative local implant of sustained-release fluorouracil has no effect on immunity in early post-operative patients with colorectal carcinoma.
