利用全身^(18)F-FDGPET/CT查找不明原因的颈部淋巴结转移癌原发灶

摘　　要：目的探讨利用全身18F-FDGPET/CT查找不明原因颈部淋巴结转移癌原发灶的临床应用价值。方法对96例原发灶不明的颈部转移癌患者行全身18F-FDGPET/CT检查,分别对单独PET图像、单独CT图像及PET/CT融合图像进行判读,所得结果与病理和/或随访结论比较。结果 PET/CT对原发灶检出率为64.6﹪(62/96),单独PET检出率为37.5﹪(36/96)及单独CT的检出率为22.9﹪(22/96),(c2值分别为9.24和16.17,P<0.01)。37.5﹪患者(36/96)经PET/CT检查后发现分期改变。PET/CT、单独PET及单独CT假阳性率分别为6.3﹪(6/96)、18.8﹪(18/96)及7.3﹪(7/96)。结论 18F-FDGPET/CT全身显像对于寻找原发灶不明的颈部转移癌患者、了解肿瘤生物学特性及全身侵犯情况有重要的临床价值,有利于临床及时制定或调整治疗方案。Objective To retrospectively evaluate whole body 18F-FDG PET/CT in depicting the primary lesion in cervical metastatic carcinoma of unknown primary site(CCUP).Methods 96 CCUP patients was included and underwent whole body 18F-FDG PET/CT imaging to search the primary lesion.A radiologist and a nuclear medicine specialist performed blinded interpretation of PET alone,CT alone and dual-modality PET/CT imaging.Results To summarise,in our patients the detection efficacy of dual-modality PET/CT was higher than PET alone and CT alone.The sensitivity was 64.6﹪(62/96),37.5﹪(36/96) and 22.9﹪(22/96) respectively.Statistical results showed the sensitivity of dual-modality PET/CT was higher than PET alone and CT alone(c2 value is 9.24 and 16.17,P < 0.01)).The staging evaluation of 36 patients(37.5﹪,36/96) was changed after whole body 18F-FDG PET/CT imaging.The false positive rate(FPR) of dual-modality PET/CT,PET alone and CT alone is 6.3﹪(6/96),18.8﹪(18/96) and 7.3﹪(7/96) respectively.Conclusion Whole body 18F-FDG PET/CT may be of relatively higher value in identifying the primary tumor of CCUP patients compared with conventional imaging methods.It also may reveal tumor biology characteristic and display tumor involvement extent.
