伽玛刀治疗原发性三叉神经痛疗效分析

摘　　要：目的研究伽玛刀治疗三叉神经痛疗效及疗效影响因素。方法 2008年1月~2012年05月收治的治疗原发性三叉神经痛48例患者,伽玛刀治疗靶点在三叉神经入脑桥段(REZ段),单靶点和双靶点中心剂量均为80~85Gy。结果伽玛刀治疗后6个月有效率最高79.2%,其后有下降趋势,病程长短、REZ显示是否清晰及双侧三叉神经粗细差别是影响疗效的重要因素,与三叉神经显示长度、治疗靶点数无相关性。结论伽玛刀治疗原发性三叉神经痛安全有效,要严格按照操作规范尽早治疗。Objective Our research attempts to discuss the effects of treating trigeminal neuralgia with γ-knife and the factors affecting the therapeutic effects. Methods 48 patients suffering from trigeminal neuralgia were treated by γ- knife from January 2008 to May 2012. There were single therapeutic target and double targets at brain root entry zone(REZ) of the trigeminal nerve. Both of them, center dose was 80~85Gy. Results The efficiency was highest(79.2%) at 6 months after treatment, and decreased beyond 6 months. The course of disease, display of REZ and thickness of the nerve were important affecting factors. The length of nerve and number of treatment targets had no correlation with therapeutic effects. Conclusion Gamma knife treatment of trigeminal neuralgia is safe and effective. We need treat patients by standard and earlier.
