应用黛力新与氯米帕明治疗神经症患者的定量效果评估

摘　　要：目的:评价黛力新治疗神经症的疗效。方法:选择60例神经症患者,随机分为黛力新治疗组和氯米帕明对照组,治疗并随访4周,在治疗前和治疗后1,2,4周进行汉密尔顿焦虑量表(HAMA)和抑郁量表(HAMD)、焦虑自评量表(SAS)和Zung量表的测定,并于治疗前和治疗4周后进行事件相关电位P300的测定,同时进行不良反应观察。结果:黛力新治疗组治疗1周后HAMA,HAMD,SAS和Zung量表评分明显下降(P<0.05)。而氯米帕明对照组各量表评分在治疗2周后开始明显下降(P<0.05)。黛力新治疗组和氯米帕明对照组P300潜伏期在治疗4周后分别为(315.64±14.11),(320.22±15.20)ms,与治疗前(347.32±15.26),(350.46±18.74)ms相比,均明显缩短(P<0.05)。氯米帕明的抗胆碱能副作用较黛力新重。结论:黛力新和氯米帕明治疗神经症均有效,黛力新在起效快、服用方便、副作用小等方面较氯米帕明有优势。AIM:To evaluate the efficacy of deanxit in the treatment of neurosis. METHODS: Sixty patients with neurosis were randomly divided into deanxit treat ment group (treatment group) and clomipramine group (control group), they were t reated and followed up for 4 weeks, and were evaluated by Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), Self-rating Anxiety Scale (SAS) and Zung Scale before and 1, 2 and 4 weeks after treatment, respectively. Event rela ted brain potential (P300) was determined before and 4 weeks after treatment, me anwhile, side effects were observed. RESULTS: The scores of HAMA, HAMD, SAS and Zung Scale were obviously decreased in the treatment group 1 week after treatment (P< 0.05), while the scores in th e control group began to decrease 2 weeks after treatment (P< 0.05). The latenci es of P300 4 weeks after treatment were 315.64±14.11 and 320.22±15.20 in the t reatment and control groups, significantly shortened compared with those before treatment (347.32±15.26 and 350.46±18.74) (P< 0.05).The anti-cholinergic side effect of clomipramine was more serious than that of deanxit. CONCLUSION:Deanxit and clomipramine are both effective in treating neurosis. C ompared with clomipramine, deanxit has the advantages of faster effect, simpler prescription and fewer side effects.
