颌下腺癌45例回顾性研究

摘　　要：目的　探索颌下腺癌的治疗效果与其病理类型、临床分期和治疗方法的关系 ,提高颌下腺癌的临床治疗效果。方法　对 45例颌下腺癌的生存率及其病理类型、临床分期和治疗方法进行回顾性研究。结果　颌下腺癌最常见为腺样囊性癌 ,其次为粘液表皮样癌。各病理类型的 3年生存率为腺样囊性癌 82 3 %,粘液表皮样癌 87 5 %,恶性混合瘤60 0 %,未分化癌 42 8%。各临床分期Ⅰ、Ⅱ、Ⅲ、Ⅳ期病例 5年生存率分别为 71 4%、63 6%、45 5 %和 3 3 3 %。不同治疗方法的 3年生存率 :局部切除肿瘤为 45 5 %,同时行颈淋巴结清扫术为 62 5 %;颌颈联合根治及术后放射治疗和化学治疗为 84 6%。结论　颌下腺癌的病理类型、临床分期和治疗方式是影响预后的主要因素 ;强调早期诊断、广泛切除后给予放疗和化疗。Objective To investigate the relationship of therapeutic efficacy with histopathological types, clinical stages, and treatment approaches in patients with submandibular gland carcinoma and to improve the survival rate of patients. Methods The survival rates, histopathological types, clinical stages, treatment approaches of 45 patients with submandibular gland carcinoma were studied retrospectively. Results The 3 year survival rates of patients with adenoid cystic carcinoma, mucoepidermoid carcinoma, malignant mixed tumor, and undifferentiated carcinoma were 82.3%,87.5%, 60.0%, and 42.8%, respectively. The 5 year survival rates were 71.4% for stage Ⅰ, 63.6% for stage Ⅱ, 45.5% for stage Ⅲ, and 33.3% for stage Ⅳ, respectively. The 3 year survival rates for patients undergoing local excision combined with radical neck dissection were 45.5% and 62.5%, respectively. The survival rate of patients undergoing radical neck dissection combined with postoperative radiotherapy and chemotherapy amounted to 84.6%. Conclusion Histopathological types, clinical stages, and treatment approaches play important roles in influencing the therapeutic efficacy of patients with submandibular gland carcinoma. Early diagnosis and radical resection followed by radiotherapy and chemotherapy should be emphasized.
