摘 要:目的:探讨并分析正常中国人垂直视动性眼震(VOKN)的临床值。方法:利用全视野眼球运动检查仪记录了27例(54眼)在3组不同模式刺激下的VOKN波形,并通过电脑进行分析结果:30°/s刺激速度时向上和向下的视动性眼震(OKN)增益值无统计学差别,60°/s OKN表现出下上的不对称性(有统计学意义)。正弦刺激下的上下OKN增益并没有表现出统计学差异。结论:VOKN的正常与否可帮助我们了解神经通路是否正常。而且垂直视动刺激是影响前庭功能反应特点的重要因素,在空间知觉形成、运动病的发生以及临床视眼动系统和前庭系统功能评定中有重要价值。AIM: To explore and analyze the characteristics and clinical values of vertical optokinetic nystagmus (VOKN) in normal Chinese subjects.· METHODS: Full-field VOKN were recorded in 54 eyes of 27 normal Chinese subjects (aged from 18 to 32 years old) using VTS-III visual vestibular function test apparatus. Each eye was given three groups of optokinetic stimulations separately at intervals of ten minutes, which were black-and- white stripes moving at 30, 60 deg/s(constant) and sinusoidally modulated wave forms(with peak speed 60 deg/s and frequency 0.05Hz) in an upward or downward direction. The data were analyzed by special software to obtain all parameter values, which were then processed with SPSS software package.· RESULTS: The gains for downward OKN (elicited by downward stimulus) and upward OKN at 30 deg/s were 0.99±0.29 and 0.97±0.29 respectively, and there was no statistic difference between the two results. When the stimulating speed was increased to 60 deg/s , the gains for downward and upward OKN decreased obviously, being 0.57±0.21 and 0.47±0.21 respectively. Moreover, these data revealed a statistical difference. As for sinusoidal OKN, the gains of downward and upward OKN were 0.64±0.37 and 0.70±0.31 respectively, and there was no significant difference between the data.· CONCLUSION: VOKN helps to provide information about the neurological pathway. The function of vertical optokinetic stimulus is also an important factor to affect the vestibular function response. It is useful in the formation of spatial perception, the occurrence of motion sickness and clinical assessment of optokinetic system and vestibular function.
