LASIK前后角膜厚度对眼压测量的影响

摘　　要：目的:研究LASIK手术前后角膜厚度改变与眼压值的关系。方法:我们从4086眼随机选择872眼在LASIK术前术后用非接触眼压计测量,应用华西医科大学PEMS统计软件包进行U 检验分析。结果:LASIK术前术后经统计分析有非常显著性差异,P <0.001,说明术后角膜厚度变薄,眼压较术前显著降低。结论:LASIK手术使膜基质层变薄,对外界压力所产生的抵抗力相应减弱,使一定的气体脉冲力压平变薄了的中央角膜所需的时间明显减少,眼压测量值就偏低。但临床医生需注意应用矫正眼压的公式计算和分析眼压时考虑角膜厚度对眼压测量值的影响以免对青光眼诊断的误诊。AIM: To explore the relation between the change of corneal thickness before and after LASIK and the applanation tonometry.· METHODS: A total of 872 eyes were randomly selected from 4 086 eyes and their intraocular pressure were measured with non-contact corneal tonometer. The data underwent U test by PEMS statistical software (West China Medical University).· RESULTS: Measured values before and after LASIK were siginificantly different (P<0.001), which indicated that the corneal stroma had been weakened and intraocular pressure decreased markedly after LASIK.· CONCLUSION: LASIK can weaken the corneal thickness and its resistance against outside pressure decreased accordingly, which cause the notable decrease of time for gas pulse to press the weakened central cornea. Then the measured intraocular pressure became lower. Therefore proper formula should be applied in clinic to correct intraocular pressure, and the effects of the corneal thickness on its measured value should be taken into consideration so as to avoid the misdiagnosis of glaucoma.·
