摘 要:通过对手术切除的 37例胃癌患者的离体标本的自体荧光光谱检测 ,显示 30 8nm,337nm激发光谱均呈双主峰结构 ,双主峰的比值均能区分胃癌与正常胃壁 (P<0 .0 0 1 )。分别用 30 8nm,337nm激发荧光光谱单因素分析诊断胃癌的灵敏度、特异度、阳性预测值分别为 88.8% ,86 .8% ,86 .4 %和 80 .1 % ,72 .1 % ,6 7.5 %。对 30 8nm及 337nm激发荧光光谱双因素分析 ,癌诊断的灵敏度、特异度、阳性预测值分别为 94 .4 % ,92 .1 % ,91 .9% ,其诊断正确符合率明显高于单因素分析的结果。研究表明 ,以 30 8nm及 337nm双波长激发检测胃壁浆膜 ,双因素判别算法可较好地区分胃癌 。A cancer diagnostic algorithm, combined with light induced autofluorescen ce spectroscopy using double excitation wavelengths, was employed for distinguishing between cancerous and normal gastric serosa. For emission spectra by 308nm excitation wavelength, the ratio between the area under 360~370nm and that under 465~475nm was calculated. In the same way, for emission spectra by 337nm excitation wavelength, the ratio between the area under 380~390nm and that under 465~475nm was calculated. The canonical discriminant function was used in the double excitation wavelength algorithm to determine a dividing line between the normal and cancerous tissues. The results showed that better performance up to sensitivity 94.4% , specificity 92.1%,and positive predictive value 91.9% could be achieved by using double excitation wavelength. The present study can be useful as a basis for further investigation in vivo autofluorescence measurement and analysis using double excitation wavelength.
