摘  要:作者探讨了核仁组成区嗜银蛋白(AgNOR)的数量、分布与乳腺癌预后的关系。乳腺肿瘤手术标本共101例,其中良性病变13例、乳腺癌88例。乳腺癌中有腋下淋巴结转移者39例,AgNOR均值为5.20±1.98,而淋巴结转移阴性者29例,AgNOR均值为4.69±2.70,二者比较(P>0.05)。各类组织类型的AgNOR值之间无明显差别。乳腺癌Ⅰ级AgNOR为3.90±1.29,Ⅱ级为5.13±2.33,Ⅲ级为6.30±2.56,随着级别的升高AgNOR值有升高的趋势,经统计学处理,Ⅰ与Ⅱ,Ⅱ与Ⅲ之间有明显差别。随访的55例中AgNOR值>5者20例,在11月~62月内死亡者14例(70%),而AgNOR值≤5者35例,在9~63月死亡者10例(28.6%),两者相差非常显著(P<0.01),而AgNOR与生存率的关系,AgNOR>5者,也明显低于≤5者,时序检验相差非常显著(P<0.01),结果表明,使用AgNOR计数除了在乳腺癌的恶性程度上提供辅助的信息之外,可能还有直接的预后价值。In a study of 101 breast tumors, including 13 benign and 88 carcinomas, the Argyrophilic NOR-associated proteins (AgNOR) were measured. The results showed that AgNOR count was 5.20±1.98 in 39 positive lymphonodes carcinomas, while it was 4.69 ±2.70 in negative lymphonodes carcinomas. There was not significant difference between the two counts (P>0.05). There was no difference between various historical types neither. But it was found that there was a trend that AgNOR counts would increase as the increase of the historical grade and there was a significant difference (P<0.05). And the follow-up results showed that 14 of 20 (70%) carcinomas whose AgNOR counts were more than 5 died during 11-62 months; but 10 of 35 (28.65%) carcinomas whose AgNOR counts were less than (or equal to) 5 died during 9 ~ 63 months. There was a significant difference between the two groups. It was also found that survival probability presented a very significant difference between above 5 AgNOR counts and under 5 counts (P<0.01). The results indicate that AgNOR counts could have a prognostic value except its assisted pathologically diagnosis for breast carcinomas.
