腰穿注入生理盐水促使小脑幕裂孔疝复位

摘　　要：目的　探讨一种简便安全有效的促进脑疝复位的方法。方法　随机选取脑疝患者 3 8例(试验组 ) ,在血肿清除术中或术毕早期即行腰穿 ,于脊髓蛛网膜下腔注入生理盐水 ,增加幕下压力 ,促使疝入小脑幕裂孔的脑组织复位。结果　早期腰穿注入生理盐水的试验组治疗效果明显好于对照组 (P<0 .0 5 )。结论　该治疗方法简单、安全 ,使脑疝病人能尽早脱离危险期 ,减少因脑疝恢复较慢而引起的各种并发症 ,促进脑功能恢复 ,是对手术的一种较好辅助治疗手段。Objective To investigate a safe and effective way in the treatment of tentorial herniation.Methods After removal of intracerebral hematoma,the tentorial herniation in 38 patinets was treated by lumbar puncture and the injection of normal saline into subarachnoid space of spinal,in order to elevate pressure in tentorial and shift the cerebral tissue to the normal site.Results The therapeutic effect in treatment group was significantly superior to that in control group (P<0.05).Conclusion This therapy is safe and simple,which is helpful to the operation and is the key for improving the therapeutic effect and prognosis.
