HA中性大孔树脂血浆吸附治疗肝性脑病的疗效观察

摘　　要：目的　观察HA中性大孔树脂血浆吸附治疗重型肝炎肝性脑病的治疗效果 ,探讨肝性脑病新的治疗方法。方法　治疗组 4 3例肝性脑病患者 ,经HA血浆吸附治疗 91次 ,平均 2 1次 ,比较治疗前后肝性脑病严重程度、血常规、血氨、凝血功能等指标 ;以 32例同类病人作为对照组。结果　HA中性大孔树脂血浆吸附治疗Ⅱ°肝性脑病较对照组有差异 ,治疗Ⅲ°2 4h内肝性脑病较对照组也有显著性差异 ,对清除血氨有较好的治疗效果。治疗安全性方面 ,观察到血小板减少 ,但对血细胞、部分凝血功能指标、患者的血压等无明显影响。结论　中性大孔树脂血浆吸附治疗肝性脑病具有较好的应用价值 ,且具有安全性高的特点。Abstract Objective The aim of this study was to evaluate the therapeutic effectiveness of plasma adsorption with HA microporous resin in patients of severe hepatitis with hepatic encephalopathy.Methods The present study randomized 75 patients of severe hepatitis with hepatic encephalopathy into plasma adsorption group of 43 patients and the remaining 32 patients served as control.91 sessions of plasma adsorption were performed with average of 2.1 treatments per patient, all were evaluated clinically in grade of HE(hepatic encephalopathy),and biochemically in hemogram, ammonia lever and coagulopathy before and after treatments.Results Plasma adsorption with HA microporous resin resulted in remarkable therapeutic effects on HE of grade 2 or 3 developed within 24 hours,with effective removal of ammonic improvement of brain oedema and SIRS).Despite of the decrease of platelet count, there were no significant changes in coagulation parameters and blood pressure.Conclusion Plasma adsorption technique with HA microporous resin can be applied safely as therapeutic management for HE.
