摘　　要：目的　探讨可解脱弹簧圈结合游离纤毛铂金弹簧圈栓塞颅内大型动脉瘤的安全性和有效性。方法　可解脱弹簧圈结合游离纤毛铂金弹簧圈栓塞颅内大型动脉瘤 4例 ,动脉瘤直径 1 4～ 2 1mm ,首先用可解脱弹簧圈在动脉瘤内成篮 ,然后用游离纤毛铂金弹簧圈进行篮内填塞 ,最后再用可解脱弹簧圈进行瘤体及瘤颈的致密栓塞。结果　 3个动脉瘤获得完全致密栓塞 ,1个动脉瘤有瘤颈残留。临床随访 3～ 1 2个月 ,无出血及脑缺血性事件发生。结论　可解脱弹簧圈结合游离纤毛铂金弹簧圈是一种可选的栓塞颅内大型动脉瘤安全。Objective To evaluate the safety, efficacy and economy of the embolization of intracranial large aneurysm by detachable coils combined with free fiber coated platinum coils. Methods Four large intracranial aneurysms were treated by detachable coils plus free fiber coated platinum coils. The diameters of the aneurysms rarged from 14 to 21 mm. Detachable coils were firstly used to form a supportive basket, then free coils were introduced for further intrabasket filling and lastly detachable coils were placed again to complete the dense packing and covering of the aneurysm neck. Results Total occlusion was achieved in 3 aneurysms and subtotal occlusion in 1 aneurysm with slight neck leakage. There were no bleeding or cerebral ischemic attack for all patients undergoing clinical follow up for 3 to 12 months after the procedure. Conclusions Embolization of large or giant intracranial aneurysms by detachable coils plus free fiber coated platinum coils should be a safe and effeclive method of choice.
