老年人胆囊结石合并糖尿病55例外科治疗

摘　　要：目的:探讨老年人胆囊结石合并糖尿病的外科治疗措施。方法:对我院1998年1月～2004年4月外科治疗的55例老年胆囊结石合并糖尿病人作回顾性的分析。结果:55例病人中,46例择期手术,空腹血糖控制在7.0～10.0mmol/L;9例急诊手术,空腹血糖控制在8.3～13.9mmol/L。术后并发症发生率:切口感染3例(5.45%),心律失常4例(7.27%),泌尿系感染2例(3.63%),肺部感染1例(1.81%)。无酮症酸中毒,无死亡病例。结论:对老年胆囊结石合并糖尿病者只要围手术期严格控制血糖,选择合理的手术时间及麻醉方式,完全可以达到理想的外科治疗效果。Objective To investigate the surgical treatmeat for elderly patients with cholelithiasis associated with diabetes mellitus . Methods From Jan.1998 to Apr .2004 ,55 elderly patients with cholelithiasis associated with diabetes mellitus undergoing surgical treatment were retrospectively studied . Results Of the 55 patients ,46 cases were subjected elective operations. Fasting blood-glucose concentration was controlled between 7.0～10.0mmol/L ;The remaining 9 cases received emergency operations. Fasting blood-glucose concentration was controlled between 8.3～13.9mmol/L.The morbidity of postoperative complications:wound infection in 3 cases (5.45%) ,cardiac arrhythmias in 4 cases (7.27%) ,uninary tract infection in 2 cases (3.63%) and pneumonia in 1 case (1.81%). There was no postoperative diabetic ketoacidosis and operative death. Conclusion If the elderly patients' blood-glucose concentration perioperatively could be controlled ,the opportunity of operation and anesthesia selected reasonably ,the effect of surgical treatment on cholelithiasis associated with diabetes would be very gratified.
