血清S100蛋白与TPS检测在恶性黑色素瘤中的临床意义

摘　　要：目的　研究血清S10 0蛋白、TPS(组织多肽特异性抗原 )在恶性黑色素瘤 (恶黑 )中的表达情况及其临床价值。方法　ELISA法动态检测 4 2例恶黑患者血清S10 0蛋白和TPS水平。结果　血清S10 0蛋白灵敏度 (Sensitiv ity ,Se)为 35 .7% ,特异度 (Specificity ,Sp) 95 .2 % ;TPSSe为 19.1% ,Sp 94 .2 %。联合检测S10 0蛋白和TPS ,平行试验 (paralleltest,两者中任何一个阳性即认为诊断试验阳性 )评价 :Se 4 7.6 % ,Sp 89.4 % ;系列试验 (serialtest,两者同时阳性 ,诊断试验才认为是阳性 ) :Se 7.1% ,Sp 10 0 % ;病情进展 ,血清S10 0蛋白血清水平和灵敏度增高 ;治疗前血清S10 0蛋白水平高于正常的患者 ,对治疗的反应较差 ;血清S10 0蛋白水平进行性升高提示复发或转移及不良预后 ,TPS在这些方面作用不明显。结论　平行试验评价血清S10 0蛋白和TPS有助于恶黑诊断 ,动态测定血清S10Objective To investigate the expression and clinical value of serum S100 protein as well as TPS(tissue polypeptide specific antigen)in malignant melanoma.Methods The levels of serum S100 protein and TPS were detected developmentally with ELISA. Results The diagnostic Se of serum S100 protein and TPS were 35.7% and 19.1% respectively,and the Sp were 95.2% and 94.2% respectively in 42 patients.The Se and the Sp were 47.6% and 89.4% respectively by parallel test,The Se and the Sp were 7.1% and 100% respectively by serial test when the patients were detected combining serum S100 protein with TPS.The level and sensitivity of the serum S100 protein became higher when the disease advanced.And the level of serum S100 protein higher than the normal before treatment could foretell poor reaction to the therapy.The patients owned higher levels would get less effort. It also indicated relapse,metastasis and poor prognosis when the level of serum S100 protein got higher in progress.However the effect of TPS was not obvious.Conclusion Combining serum S100 protein and TPS by parallel test can connect to the diagnosis of malignant melanoma,and the levels of serum S100 protein rising progressively can reveal the change of melanoma.
