肝硬化难治性腹水的超滤浓缩回输治疗

摘　　要：目的　研究腹水超滤浓缩回输治疗难治性腹水的疗效。方法　利用FCN 0 1型腹水超滤浓缩回输机及DY 10 6 0一次性腹水浓缩器对 2 2例难治性腹水患者在抽取腹水浓缩后进行静脉回输。结果　显示症状改善 ,腹水明显消退 [治疗前后腹围为 (89± 8)cm、(78± 6 )cm ,P <0 .0 5 ];血浆白蛋白升高 [治疗前后为 (2 5 .2± 7.2 ) g/L、(36 .3± 6 .7)g/L ,P <0 .0 5 ];每 2 4h尿量显著增加 [治疗前后为 (6 4 0± 4 0 )ml、(12 0 0± 6 0 )ml/,P <0 .0 1];血尿素氮和肌酐则下降 [治疗前后分别为 (9.3± 3.8)mmol/L、(5 .8± 3.0 )mmol/L和 (14 9.2± 2 6 .3) μmol/L、(89.5± 17.2 )μmol/L ,P <0 .0 5 ];腹水中蛋白质明显上升 [治疗前后为 (5 .2 3± 1.2 )g/L、(2 2 .14± 1.5 ) g/L ,P <0 .0 1]。Objective To investigate the therapeutic efficacy of ultrafilter condensation in obstinal ascites.Methods The ascites from cirrhotic patient was condensed and then transported into the patient him/her-self with type FCN-01 machine of ultrafilter condensation and importation.Results Compared to pre-transportion the symptoms were improved,ascites was obviously reduced(abdomen-cycle:89±8 cm VS (78±6)cm,P<0.05);plasma albumin was increased(25.2±7.2)g/L VS (36.3±6.7)g/L,P<0.05);urine amount was significantly increased(640±40)ml VS (1200±60)ml,P<0.01);levels of BUN and Scr were declined(9.3±3.8)mmol/L VS (5.8±3.0)mmol/L and (149.2±26.3)μmol/L VS 89.5±17.2μmol/L, respectively,P<0.05);The protein in ascites was remarkably increased(5.23±1.2)g/L VS (22.14±1.5)g/L,P<0.01).Conclusion Ultrafilter condensation and importation of ascites is a practical and effective method for treating obstinate ascites.
