消化道神经内分泌癌10例临床病理分析

摘　　要：目的　探讨消化道神经内分泌癌的临床病理特征 ,对其诊断和鉴别诊断进行讨论。方法　报道 10例消化道神经内分泌癌 ,结合文献从临床病理 ,免疫组化等方面进行探讨。结果　 10例中 7例临床经过呈进行性 ,侵袭性。镜检 4例为不典型类癌 ,6例为小细胞神经内分泌癌。肿瘤细胞CK、NSE、CgA、Syn均呈阳性表达。 结论　消化道神经内分泌癌具有复杂的组织学特点。根据肿瘤组织及细胞的异型性 ,可分为高 ,中 ,低分化。正确的组织学分类有助于临床正确治疗和预后判断。Objective To study the clinicopathological features of gastrointestinal neuroendocrine carcinoma (GNC),and to elucidate its diagnosis and differential diagnosis.Methods clinicopathologic analysis and immunohistochemical staining were performed in 10 cases of GNC. Results: The course of the disease was aggressive in 7 of the 10 cases. Histologically, there were 4 cases of atypiacal carcinoid and 6 of small cell type. The tumor cells showed a positive test for CK、NSE、CgA and Syn.Conclusion GNC Has a pleomorphic appearance of histological features with well, moderate and poor-differentiated areas. Histological type of the tumor is important for clinical therapy and prognostic evaluation. Immunohistochemical staining is helpful in differential diagnosis of GNC from other gastrointestinlal tumors.
