摘  要:目的:探讨血管性痴呆(vasculardementia,VD)与脑脊液中生长抑素、精氨酸加压素(AVP)和阿片肽β-内啡肽(β-EP)和神经元特异性烯醇化酶(neuron-specificeno-lase,NSE)的相关性及临床意义。方法:运用简易精神状态量表、Hachinski缺血量表和社会功能活动调查评定患者认知功能,用放射免疫分析法(RIA)测定脑脊液中生长抑素、AVP,β-EP的含量,用双抗体免疫夹心法(ELISA)测定脑脊液中NSE的含量。结果:脑脊液中生长抑素、AVP,β-EP,NSE含量,在VD组、无痴呆脑梗死组和对照组间差异均有显著性意义(P均<0.01),而无痴呆脑梗死组脑脊液中AVP(4.98±0.43)ng/L,NSE(22.36±11.42)ng/L显著增高(P<0.01);VD组生长抑素中生长抑素、AVP,β-EP,NSE含量随痴呆程度的加重,呈递减的趋势(P均<0.01)。结论:VD与脑脊液中生长抑素、AVP,β-EP,NSE的含量下降呈正相关,对相关指标的监测有助于血管性痴呆的早期诊断,亦可作为判断病情轻重的重要依据。AIM:To investigate the correlation of vascular dementia(VD) with somatostatin(SS) ,argvasopressin(AVP),beta endorphin(β EP),neuron specific enolase(NSE) in cerebrospinal fluid(CSF) and the clinical significance. METHODS:Mini mental state examination(MMSE) , Hachinski ischemic scale (HIS) and function active questionnaire(FAQ) were adopted to valuate the cognitive function of patients.The radioimmunoassay(RIA)was carried out to test the contents of SS,AVP and β EP.Enzyme linked immunoadsorbent assay(ELISA) was used to test the contents of NSE in CSF. RESULTS:The contents of SS,AVP,β EP and NSE in CSF had significant differences among VD group,CI group and control group(P < 0.01),while the levels of AVP[(4.98±0.43) ng/L],NSE[(22.36±11.42) ng/L] in CSF increased significantly in CI group(P< 0.01) .The contents of SS,AVP,β EP and NSE in CSF in VD group had a decreasing trend along with the dementia development(P< 0.01). CONCLUSION:VD has positive relativity with the decreased contents of SS,AVP,β EP and NSE decreasing in CSF.The monitoring of relative indexes is not only helpful to the early diagnosis of VD,but also can serve as a basis for diagnosing the degree of disease.
