非小细胞肺癌支气管镜下表现与pTNM分期的关系

摘　　要：目的 :了解中心型非小细胞肺癌支气管镜下改变与 pTNM分期的关系。方法:用纤维支气管镜对86例中心型非小细胞肺癌进行观察 ,并将其镜下表现与病理结果进行比较研究。结果:pT1 期16例 ,pT2 期70例 ;pN0期41例 ,pN1～2 期45例。支气管镜下发现支气管腔内肿瘤周围黏膜无改变者 (Ⅰ型 )36例(41.86 %) ,肿瘤周围黏膜充血、水肿、粗糙等改变者 (Ⅱ型 )50例(58.14%)。Ⅰ型、Ⅱ型在 pT1 期中的发生率分别为68.75 %和31.25 % ,在pT2 分别为35.71 %和64.29 % (χ2=5.84,P<0.05) ;在 pN0 期发生率分别为58.54%、41.46% ,在 pN1～2 分别为26.67 %和73.33 % (χ2=8.95 ,P<0.01)。结论:中心型非小细胞肺癌支气管镜下改变与 pTNM分期有关 ,pT1 和pN0 期以Ⅰ型多见 ,pT2 和 pN1～2Objective:To observe the relationship between tumor feature under bronchoscopy and pTNM stages in central type non-small cell lung cancer.Methods:Bronchoscopic examinations were completed in86patients with central type non-small cell lung cancer who underwent curative pulmonary resection.Results:Sixteen patients were pT 1 stage,70were pT 2 stage,41were pN 0 stage,and45were pN 1～2 stage.Under bronchoscopy,typeⅠwhich was defined as the appearance of localized polypoid and no invasion to the wall of the surrounding bronchi was in36patients,and typeⅡwhich was defined as the abnormal mucosa invicinity was in50patients.The patients with typeⅠand typeⅡin pT 1 and pT 2 stage were11(68.75%),5(31.25%),25(35.71%),and45(64.29%),respectively(χ 2 =5.84,P<0.05).The patients with typeⅠand typeⅡin pN 0 and pN 1～2 stage were24(58.54%),17(41.46%),12(26.67%),and33(73.33%),respectively(χ 2 =8.95,P<0.01).Conclusion:The tumor form was correlated with pTNM stages in central type non-small cell lung cancer.The majority of pT 1 and pN 0 stages were typeⅠ,and the majority of pT 2 and pN 1～2 were type.
