摘  要:目的评估内镜辅助下经外侧裂入路对基底节区高血压性脑出血手术治疗的疗效。方法将44例高血压性基底节区脑出血病人随机分为两组,分别采用经外侧裂入路内镜辅助下显微神经外科技术清除血肿(经侧裂组)和经皮质造瘘清除血肿(经皮质组),经皮质组部分借助显微神经外科技术。结果经侧裂组在血肿清除程度、术后神经功能恢复、残废率、病死率等方面均明显优于经皮质组。结论经外侧裂入路内镜辅助手术治疗基底节区高血压性脑出血是一种损伤轻、疗效优、预后好的微创治疗方法。objective To evaluate the therapeutic effects of the hypertensive hematomas in basal ganglia region by transsylvian fissure with endoscopic-assisted microneurosurgery. Method 44 cases with hypertensive hematomas in basal ganglia regions were randomized into two groups. The first group patients were treated by transsylvian fissure approach with endoscopic-assisted microneurosurgery. The second by transcortical approach, most of them with microneruosurgery. Results The first group patients had all advantages better than the second group in respects of the clearance rate of the hematomas, recovery of the neurological deficits, disability degree and mortality. Conclusions Transsylvian fissure microsurgical treatment for hypertensive intracerebral hematoma in basal ganglia region with endoscopic-assisted microneurosurgery is a kind of minimally invasive method with the less damage, better effects and more favorable prognosis.
